Psammomatoid juvenile ossifying fibroma (PJOF) is a rare benign tumor that usually affects the paranasal sinuses, orbit, and skull. In most cases, extensive incisions are necessary for full access to the tumor site. The aim of this paper is to report a case of extensive PJOF in which an intraoral surgical approach was performed with complete excision of the tumor. A female patient, 18-year old had a deforming volume increase in the region of the left facial middle third with an approximate evolution time of 2 years. She complained of headache, epiphora in the left eye, and total obstruction of the left nostril. Extraoral examination showed facial asymmetry with dystopia, ocular proptosis, and considerable sclera exposition of the left eye. Tomography examination showed a mixed aspect lesion on the left side of the face, well delimited. The intraoral surgical approach was chosen for the excision of the lesion. Under general anesthesia and nasotracheal intubation, total resection was performed, followed by exodontia of the directly involved teeth. In the definitive histopathological examination, the diagnosis was confirmed. The patient is currently with 2 years and 6 months of postoperative follow-up and has good healing of intraoral surgical wounds and stable occlusion. Extraoral examination showed harmonic projection of the facial middle third, but still with excessive exposure of the sclera of the left eye due to the defect in the orbital floor. The surgical treatment of PJOF was possible by intraoral approach, even taking into account the limitations of the access and the complexity of the anatomy of the facial bones involved.
The computed anesthetic technique showed lower mean pain perception, but did not show statistically significant differences when contrasted to the conventional technique.
Introdução: O uso de equipamentos de proteção individual (EPI) por motociclistas é de grande importância para a prevenção das consequências decorrentes de acidentes. Políticas no Brasil enfatizaram a necessidade do uso de tais equipamentos. O objetivo do estudo foi estimar a frequência e associação do uso de EPI entre os motociclistas e oferecer conselhos sobre o uso adequado. Métodos: Estudo transversal, com uma amostra de 1.516 motociclistas na região metropolitana de Recife, Brasil, entre julho de 2014 e junho de 2015. Foi realizado um questionário sobre gênero, idade, aceitação do teste de alcoolemia, o uso de EPI e cilindrada da motocicleta. Utilizou-se as estatísticas inferenciais ANOVA e teste post-hoc de Tukey. Resultados: 96% da amostra de motociclistas eram do sexo masculino. Capacetes, sapatos e jaquetas foram os EPI mais utilizados. 92% da amostra concordou em ouvir conselhos sobre o uso de EPI. Associações significativas foram encontradas entre a idade e os indivíduos que ouviram os conselhos sobre a importância dos EPI, bem como entre o número de EPI utilizados e a cilindrada da motocicleta. Conclusão: Os achados deste estudo enfatizam a importância do uso dos EPI, que são obrigatórios de acordo com a legislação brasileira.
Propouse: surgical teams sometimes neglect surgical glove perforation. The aim of this study was to determine the prevalence of sterile glove perforation in maxillofacial surgical procedures. Methods: Two studies were performed. The first one evaluated 200 pairs of surgical gloves (50 in each group) used by first, second, and third-year residents (R1,R2,R3). In the second study, 150 pairs of surgical gloves were evaluated: 100 pairs were used by a third-year resident, including 50 pairs in oral surgery procedures and 50 pairs in trauma surgeries, and 50 pairs of unused gloves were tested as control. The gloves were examined for perforations by filling them with water and testing for leaks. Results: In the first study, 29 pairs (19.3%) were perforated. Of the 50 pairs of control gloves, 7 pairs (14%) showed perforations on either side. The thumb and index finger and the right-hand (dominant hand) gloves, were the most frequently perforated sites. In the second study, 6% of the gloves in the control group, 6% of the gloves used in oral surgeries, and 10% of the gloves used in trauma surgeries showed perforations. The index finger and thumb were the most affected sites. Conclusion: The perforation rate was higher for the dominant right hand. In both studies, the frequency of perforation of unused gloves was significant, indicating the need for better quality control of the gloves evaluated in this study.
