Objective: Understand the meaning, in hospital work, of hierarchical structures present in the organizational culture of Brazilian public hospitals. Methods: The corpus of research was originated in four theses and six dissertations, and was organized, analyzed and interpreted from the perspective of symbolic anthropology interpretation. Results: Hospitals copied the professional mechanistic bureaucracy and hierarchies, from these structures, produce meanings that indicate a fragmentation of relationships, professional disputes and separations, as well as conflicts and subversive behavior at work. Conclusion: The hierarchies in each of the bureaucracies created several clashes that disrupt the workers and their work processes. Reorientation strategies and awareness of the hierarchical boundaries should be studied so that the work is optimized. A qualitative, exploratory study carried out from March to December 2002 in a university hospital based in the far south of the country, which aimed at identifying the nurses' perception on the administrative actions exercised in their daily work. Thematic categories emerged from data survey and content analysis in the interviews carried out with 10 nurses. We emphasize Category I: Administrative Actions as a Management Instrument; and Category II: Administrative Actions as Direct/indirect Care. In the first one, administrative actions are perceived as planning, coordination, leadership, control, information retention, and organization; the second one depicts administrative actions as an integral part of nursing care. It has been observed that many administrative actions exercised by nurses are inherent to their role, in keeping with the Nurse's Professional Exercise Code, and they are critical for care.
ReSUmOObjetivou-se identificar o cronotipo das enfermeiras da manhã e noite de um hospital universitário, verificar como o trabalho nestes turnos influencia seu sono e correlacionar os cronotipos com influências que o sono produz no trabalho e fora dele. Participaram 15 enfermeiras que responderam ao questionário de Horne & Ostberg e à entrevista semiestruturada entre março e julho de 2010. Os dados dos questionários foram organizados em planilhas e os das entrevistas analisados pela Análise Temática. Os resultados apontam cinco enfermeiras moderadamente matutinas, quatro indiferentes, duas moderadamente vespertinas, três definitivamente vespertinas e uma definitivamente matutina. Mostram que o trabalho em turnos faz com que as entrevistadas desenvolvam hábitos alimentares e de sono, peculiares. Conclui-se que o perfil cronobiológico das enfermeiras não traduz suas subjetividades na relação do sono com turnos de trabalho; enfermeiras da noite apresentam maiores disfunções no ciclo sono-vigília, tais disfunções repercutem igualmente no cotidiano das enfermeiras de ambos os turnos. descritores: Disciplina de Cronobiologia; Trabalho em turnos; Sono; Enfermeiras; Administração de recursos humanos em hospitais.aBStRaCt This study was aimed at identifying the chronotype of nurses in the morning and evening shifts of a university hospital, how these workshifts influence their sleep and correlate chronotypes with influences of sleep on and off the workplace. Fifteen nurses participated in the study by answering to the questionnaire of Horne&Ostberg and semi-structured interviews between March and July 2010. Data from questionnaires were organized into spreadsheets and the interviews were analyzed by thematic analysis. Nurses worked as follows: five in moderately morning shift, four in indifferent shift, two in moderately evening shift, three in evening shift and one in morning shift. Shift work was found to develop peculiar eating and sleep habits. The chronobiological profile did not translate subjectivities regarding sleep and work shifts. Night nurses demonstrated higher dysfunction in sleep-wake cycle, which had an impact on the daily lives of all nurses. Quantitative study aimed to identify the enterprising tendency of nurses at a university hospital and to relate them with age, length of work in the hospital and conclusion of the nursing course. This cross-sectional quantitative study was developed in 2010. All 60 nurses from the hospital answered the questionnaire General measure of Enterprising Tendency, which contains five categories. In the creativity category one nurse obtained two points; in need for achievement one nurse totaled 12 points; one nurse obtained two points; in motivation four nurses achieved higher scores; in taking calculated risks, the highest score was 10 points; in autonomy, nine nurses obtained one point each. Individuals aged between 27 and 33 years showed higher enterprising tendencies. Reduced enterprising tendencies were found for nurses aged between 43 and 56 years, graduated more than 17 years ago and with a greater length of work. Actions are necessary to encourage nurses in the age range of enterprising tendency decline and those who graduated longer ago and who have worked for a longer period of time in the hospital.
