AHA Assisting Hand AssessmentAIM The aim of this study was to determine the immediate effect of wearing a wrist and thumb brace on the performance of bimanual activities in children with spastic hemiplegic cerebral palsy.METHOD In a pre-and post-test cohort study of 25 children (age range 4-11y; mean age 8y 4mo[SD 2y 2mo]; 16 males, 9 females) with spastic hemiplegic cerebral palsy with a Zancolli classification hand score of I, IIA, or IIB (mild and moderate hand dysfunction; children with a Zancolli classification of III -severe hand dysfunction -were excluded from this study), performance of bimanual activities was evaluated with the Assisting Hand Assessment (AHA) on three occasions: one assessment with a static wrist and thumb brace placed on the affected hand and two other assessments without a brace. The differences between AHA scores obtained at the three assessments were evaluated using the repeated measures analysis of variance.RESULTS Performance of bimanual activities while wearing the brace improved significantly compared to performance without the brace (p<0.001). With the brace, the mean AHA score increased by 3.2 (95% confidence interval 2.1-4.3) from 59.1 to 62.3. The scores of the two assessments without the brace did not differ significantly.
This set of activities has good potential for assessment of the performance of functional activities in children with OBPL. This study, however, showed a number of difficulties in observing and scoring the activities that have to be considered when developing a standardized video observation.
SUMMARY
The authors studied the body posture at rest of 15 healthy term infants with birthweights appropriate for gestational age (AGA) and 15 infants who were small for gestational age (SGA). In both groups, there was wide inter‐individual variability in pos:ural behaviour. Apart from a preference for a position with all limbs in abduction between I and 90°, independent of the degree of flexion or extension and internal/external rotation, no dominant preference posture could be seen for either group. The SGA infants showed a more flexed position of the arms, ccrnpared with the AGA group. One of the factors contributing to this difference might be the influence of intrauterine malnutrition on the development of the central nervous system.

En dehors d'une préférence pour une position de tous les membres dans une abduction entre 1 et 90°, independante du degré de flexion, d'estension ou de rotation internelexterne, aucune préférence posturale ne put ětre relevée dans I'un ou I'autre groupe. Les nourrisons SGA montraient une position plus nechie des seuls bras, en cornparaison avec le groupe AGA. Un des facteurs contribuant á la différence pourrait ětre l'influence de la mal‐nutrition intra‐uterine sur le developpement du systeme nerveux central.

Körperhaltung bei reifen SGA und AGA Neugeborenen
Die Autoren untersuchten die Ruhekörperhaltung bei 15 gesunden, reifen Neugeborenen mit altersentsprechendern Geburtsgewicht (AGA) und bei I5 hypotrophen Kindern (SGA). In beiden Gruppen fand sich eine groβe individuelle Variationsbreite der Korperhaltung. Auβer einer Vorliebe für eine Abduktion aller Extrernitäten zwischen 1 und 90°, unabhängig vom Grad der Beugung oder Streckung und interner/externer Rotation konnte bei keiner der Gruppen eine besondere Vorzugshaltung beobachtet werden. Die SGA Kinder zeigten irn Vergleich zur AGA Gruppe häufiger nur eine Beugehaltung der Arrne. Einer der Faktoren, der zu diesern Unterschied beitrágt, ist vielleicht der Einfluβ der intrauterinen Mangelernährung auf die Entwicklung des Zentralnervensystems.
