Acute Respiratory Infections (ARI) is one cause of death in infants and toddlers. The incidence of ARI at West Sumatera in 2016 was 154, 962 cases and increased by 2017 as many as 169, 031 cases. The highest incidence of ARI was found in Padang Pasir for age <1 year 568 people and age 1-4 years 1648 people. The high number of ARI cases in children under five is due to lack of knowledge of the mother about the household environment to prevent ARI of toddlers. Sample was 86 mothers. The study was conducted by literature study technique. The result showed that 30, 2% mother knowledge about ARI still low. Activities to improve knowledge of mother toddlers need to be intensified again by way of counseling about the prevention of ARI through Posyandu cadres and health personnel so as to reduce the incidence of ARI.

Integrated Puskesmas Recording and Reporting System (SP2TP) is a comprehensive (integrated) Puskesmas recording and reporting activity with the concept of the Puskesmas working area. Based on the initial survey at the Nangggalo Health Center already using the e-Puskesmas application in the recording and reporting section only the officers still use manual reporting because of the lack of facilities in the implementation of recording and reporting and work placement not in accordance with the profession. The purpose of this research is to analyze the implementation integrated puskesmas recording and reporting system at the Nanggalo Padang Health Center in 2018. The study was conducted at the Nanggalo Health Center on 02 to 14 July 2018, the type of research conducted was qualitative with a phenomenological approach. The informants in this study were 4 people, all informants were scrutinized by using in-depth interview guidelines. Data analysis in this study uses the Colaizzi model approach. The sampling technique is purposive sampling. The results obtained are for input components such as human resources are quite good, but for SP2TP officers there is no information technology department, there is no special facility for SP2TP, and no special funds for SP2TP. For the process component, the manual recording and reporting of the SP2TP implementation is not yet available, and there is no monitoring and evaluation at the Nanggalo health center. Based on the above conclusions, the researcher suggested to the Puskesmas that there should be additional human resources for D3 graduates medical records and health information in accordance with Permenpan No. 30 of 2013, providing facilities and funds to support the implementation of SP2TP, in the process of recording and reporting should use the e-Puskesmas application and need the existence of monitoring and evaluation in the implementation of SP2TP.
The Community Health Center hereinafter referred to as the Puskesmas, is a health service facility that organizes public health efforts and first-rate individual efforts, prioritizing promotive and preventive efforts. The recording and reporting system (SP2TP) is a comprehensive and integrated recording and reporting activity for Puskesmas with the concept of puskesmas working areas. PIECES include Performance, Information, Economic, Control, Efficiency, and Service. Puskesmas Lubuk Buaya already uses the e-Puskesmas application and already has PIECES, but it is not yet perfect in its application. This community service's purpose is that the implementation of SP2TP by using PIECES is better at the Lubuk Buaya Health Center. The method used with Research Development. Medical record officers' performance on the implementation of e-puskesmas has been running well on the information is still not accurate in providing information. The economy is still lacking facilities and infrastructure in the smooth implementation of e-puskesmas. There are still errors in the data in control so that supervision is still not guaranteed; efficiency has not been found to increase efficiency in the performance of health centers and services there are still processes that have not been implemented. In the implementation of SP2TP using PECES, it should be optimized so that errors do not occur in the information available at the Puskesmas, and other fields related to reporting will be easier to monitor, including external parties.
