Sporotrichosis associated with zoonotic transmission remains a relevant public health problem in Rio de Janeiro, Brazil, affecting a large at-risk population, which includes HIV-infected individuals. We assessed patients co-infected by Sporothrix spp. and HIV over time in the context of an unabated sporotrichosis epidemic.A retrospective cohort retrieved information from a National reference institute for infectious diseases regarding 48 patients with sporotrichosis-HIV co-infection (group 1) as well as 3,570 patients with sporotrichosis (group 2), from 1987 through March 2013. Most patients from group 1 were male (68.8%), whereas women were predominant in group 2 (69.1%; p<0.0001). Patients from group 1 were younger than those from group 2 (μ = 38.38±10.17 vs. 46.34±15.85; p<0.001) and differed from group 2 in terms of their race/ethnic background, with 70.8% non-white patients in group 1 vs. 38.6% from group 2 (p<0.0001). Close to half (∼44%) of the patients from group 1 were hospitalized due to sporotrichosis over time, whereas hospitalization was very unlikely in group 2, among whom approximately 1% were hospitalized over time. Dissemination of sporotrichosis was the main cause of hospitalization in both groups, although it was more common among hospitalized patients from group 1 (19/21 [90.5%] vs. 16/37 [43.2%]; p<0.001). Over the period under analysis, eight patients died due to sporotrichosis (3/48 vs. 5/3,570). The diagnosis of sporotrichosis elicited HIV testing and subsequent diagnosis in 19/48 patients, whereas 23/48 patients were simultaneously diagnosed with the two infections.HIV infection aggravates sporotrichosis, with a higher incidence of severe disseminated cases and a higher number of hospitalizations and deaths. Underserved populations, among whom sporotrichosis has been propagated, have been affected by different transmissible (e.g., HIV) and non-transmissible diseases. These populations should be targeted by community development programs and entitled to integrated management and care of their superimposed burdens.
RESUMOA integração do ensino à pesquisa e extensão objetiva consolidar todo o processo de amadurecimento pessoal do aluno. Nesta ótica foi criado em 2013 o projeto de pesquisa/extensão: "A inserção do acadêmico de enfermagem no âmbito da pesquisa científica focando os determinantes e condicionantes do processo saúde-doença, seus desdobramentos e relações em diferentes níveis de atenção e promoção da saúde à localidade, ao indivíduo e a família". O objetivo foi descrever o uso de metodologia ativa no processoensino-aprendizagem de alunos de graduação em enfermagem através da participação em atividades de extensão dentro do currículo regular. Exploramos o relato de experiência dos projetos utilizando a metodologia da problematização. Durante três semestres foram desenvolvidas 11 atividades de extensão, envolvendo 118 alunos. Concluímos que uma vez vivenciada a experiência além dos muros da universidade foi possível desenvolver habilidades e competências importantes para seu exercício profissional, pois levará consigo uma bagagem de experiências reais.Palavras Chaves: Ensino; Enfermagem; Relações Comunidade-Instituições; Determinantes Sociais de Saúde ABSTRACTThe integration of education research and objective extension consolidate all of the student's personal maturation process. In this light was created in 2013 the research project / scope: "The insertion of the nursing academic in scientific research focusing on the determinants and conditions of the health-disease, its consequences and relationships at different levels of care and health promotion to location, the individual and the family. The aim were to describe the use of active methodology in the process-teachinglearning in nursing undergraduate students through participation in outreach activities within the regular curriculum. We explore the project experience reporting using the methodology of questioning. During three semesters were developed 11 outreach activities, involving 118 students. We conclude that once lived experience beyond the university walls was possible to develop important skills and competencies for their professional practice, as will carry a baggage of real experiences.
Objectives: to understanding the perception of Primary Health Care users about the professional identity of nurse. Methods: this is an exploratory, descriptive, cross-sectional and quantitative study, using the STROBE instrument. The sample included 94 users grouped according to the Family Health Strategy coverage. Results: the idealized identity of nurses dates back to the past of the profession. The subjects, in 81.9%, associate the female gender to the profession. 63.8% of them believed that the nurse is a subordinate, and from these, 90% believed they were subordinate to physicians. The analysis of the perception of the activities under responsibility of the nurse showed the predominance of assistance activities that are not exclusive to them. Conclusions: the findings indicate partial awareness about the identity and the role of nurses in Primary Health Care. The main deficits are related to the competences of this professional. The understanding of the role of the nurse was higher in the population under the coverage of the Family Health Strategy.
As mudanças curriculares propostas pelo Ministério da Educação para os cursos da saúde impulsionaram as universidades de enfermagem a um processo de revisão e reformulação de cursos. Pensando nisso, o objetivo deste trabalho é analisar as mudanças curriculares do curso de Enfermagem responsáveis para a determinação do perfil do Enfermeiro egresso de uma Instituição privada no Rio de Janeiro, a Unigranrio. A partir de um estudo histórico-social, que lançou mão de uma análise documental, foram verificados nove currículos entre 1981 e 2013, dentre os quais os dois primeiros se apresentam tímidos no sentido de formação de um profissional envolvido com a transformação social. Após a instituição da LDB/96, a Unigranrio passou a oferecer 32% da carga horária destinada a disciplinas que contemplam o conceito ampliado de saúde e o “cuidar” do Enfermeiro em todos os campos de atuação, além das discussões transversais nas demais disciplinas, incluindo políticas públicas de saúde, que proporcionam ao aluno a perspectiva da organização do Sistema Único de Saúde (SUS). Concluiu-se que a instituição analisada não seguiu a vertente assistencialista, mas optou pela formação de profissionais para atuar com autonomia, fazendo a diferença nas comunidades, visando um perfil de Enfermeiro com base na subjetividade e criatividade capazes de transformar a realidade na qual a Enfermagem é necessária.
