There are few estimates of influenza burden in the WHO Region for the Eastern Mediterranean. In this study we estimated the burden of severe acute respiratory infection (SARI) and influenza-associated SARI (F-SARI) in selected provinces of Islamic Republic of Iran, the trends of SARI and confirmed cases of influenza (F-SARI) over 12 months (seasonality), and the age groups most at risk. Using the electronic Iranian influenza surveillance system and data of cases in sentinel hospitals of 3 selected provinces, we estimated the monthly trend (seasonality) of incidence for SARI and F-SARI, overall incidence of SARI and F-SARI and their disaggregation by age with the aid using the Monte Carlo technique. The age groups most at-risk were children aged under 2 years and adults older than 50 years.

Purpose
The purpose of this study is to change the level of entrepreneurial alertness from individual to group and from the group to the organizational level and apply it to international market entry.
Design/methodology/approach
A literature review, meth-synthesis, interview and focal group are used for the final research framework. The way of theorizing for changing levels of entrepreneurial alertness is multi-level method. A framework for international market entry, a process of internationalization and an organizational learning process was used for the final conceptual framework.
Findings
A new framework for international entrepreneurial alertness in the opportunity discovery of individual, group and organizational level was developed.
Research limitations/implications
Researchers will be able to use this framework for extending opportunity discovery and entrepreneurial alertness theories at individual, group, organizational and international levels.
Practical implications
Using the final framework, entrepreneurs and organizations will be able to discover new opportunities, and teachers will be able to educate international entrepreneurial alertness and opportunity discovery at individual, group and organizational levels.
Originality/value
The paper begins with an overview of previous researches in this area and then moves on to combine them using meth-synthesis to create a new framework for international entrepreneurial alertness in opportunity discovery. Note that the levels of entrepreneurial alertness from individual to group and from the group to the organizational level have been changed using a multi-level theorizing method.
Chronic pain following lower-limb amputation is now a well-known neuropathic, chronic-pain syndrome that usually presents as a combination of phantom and stump pain. Controlling these types of neuropathic pain is always complicated and challenging. If pharmacotherapy does not control the patient’s pain, interventional procedures have to be taken.The aim of this study was to evaluate the efficacy of using pulsed radiofrequency (PRF) on the dorsal root ganglia at the L4 and L5 nerve roots to improve phantom pain.Two patients with phantom pain were selected for the study. After a positive response to segmental nerve blockade at the L4 and L5 nerve roots, PRF was performed on the L4 and L5 dorsal root ganglia.Global clinical improvement was good in one patient, with a 40% decrease in pain on the visual analogue scale (VAS) in 6 months, and moderate in the second patient, with a 30% decrease in pain scores in 4 months. PRF of the dorsal root ganglia at the L4 and L5 nerve roots may be an effective therapeutic option for patients with refractory phantom pain.
Classic acupuncture and YNSA are similarly effective in the prophylaxis and treatment of migraine headache and may be considered as alternatives to pharmacotherapy.
