This article presents experimentally proved scientific-practical methods of developing modern vulnerary medical preparations in the form of an ointment with antibacterial, anti-inflammatory and anesthetic properties for the purposes of military medicine. In pursuit of the designated goals, the authors used modern methods of research: pharmacological-technological, physical-chemical, structural-mechanical and biological methods – for proving the compound of the combined ointment including ofloxacin, nimesulide and lidocaine. On the basis of the research conducted, we proved the content and rational technique of the complex antimicrobial ointment with anti-inflammatory and anesthetic effect using ofloxacin (0.05 g), nimesulide (1.0 g) and lidocaine (4.0 g). We experimentally established the optimal content of the main component of the ointment: emulsive base of mixture of Polyethylene Oxide-400 and Polyethylene Oxide-1500 (1 : 4) and a complex of emulsifiers of the first and the second generations – cetostearylic alcohol – 3%, monoglyceryl stearate – 5% at the constant concentration of paraffin oil – 20%, which involves a certain temperature regime during preparation, a sequence of introducing active pharmacological ingredients and adjuvants to the water and oil phases, frequency and duration of mixing. Physical-chemical tests of the ointment base determined the optimum concentration of glycerine as 5%, which encourages elimination of purulent discharge from damaged tissues, causing an anti-inflammatory effect and providing a prolonged, but milder dehydrating effect. The relation between physical-chemical, rheological, biopharmaceutical properties of the developed preparation and the concentration and method of introduction of the active pharmaceutical ingredients was theoretically and experimentally proven. We conducted pre-clinical studies (in vivo) for defining the therapeutic efficiency of the ofloxacin-nimesulide composition in the compound of the developed ointment for complex treatment of the wound process. We studied the specific activity (anti-alternative – model of standard tissue wounds in rats), which allowed us to establish the optimum concentration of nimesulide, which is 1%. Biopharmaceutical studies (in vitro method) determined the optimum method of introducing ofloxacin, nimesulide and lidocaine in the ointment base as a solution in dimethyl sulfoxide. To select an anesthetic substance for the ointment, the authors conducted a comparative study on the anesthetic effect of the anesthetics most widely available in Ukraine: novocaine, dicainum, lidocaine, trimecainum and pyromecainum. The research conducted on the time it took for the anesthetic effect to occur and on the duration and extent of the effect enabled us to select a 4% concentration of lidocaine anesthetic for the preparation. The choice of the temperature regime for the ointment’s preparation was proved. The in vitro method and rheological studies determined that the optimum temperature regime for the technological process of preparing the ointment on the emulsive base is 25–30 ºС. The development and introduction of the new combined ointment on a contemporary hydrophilic ointment base into practical pharmacy would lead to an enlargement in the nomenclature of the medical preparations used for local treatment of the purulent – necrotic phase of the wound process, which is relevant for pharmaceutical technology and global medical science and practice.
The article is devoted to a promising direction in pharmacological mycology and biotechnology, namely the use of Basidiomycetes as a biologically active substance for preparation of pharmaceuticals. The anticancer and immunostimulating properties of Basidiomycetes result from the structural characteristics of the polysaccharides: the presence of β-(1 → 3) connections in the main chain and an additional β-(1 → 6) branched part of glucans. Attention is focused on Shiitake mushrooms (Lentinula edodes) as a unique biosystem with a strong ability to be involved in the regulatory mechanism of human immunomodulation. The purpose of the theoretical and experimental study is development of a pharmaceutical preparation from Shiitake biomass (including its immune cells), the effectiveness of which is considered from the standpoint of the chemistry of natural compounds. Having a 3000-year history of cultivation and application, this mushroom is again of interest to scientists around the world because of investigation into its therapeutic and prophylactic potentials. For the first time, the biotechnological method of cultivating L. edodes biomass was developed with the use of deep cultivation, adapted to the plant raw material base of Ukraine. Malt wort was used as a nutrient medium without processing Humulus lupulus. The analysis of research and preclinical studies of specific activity of the designated biosystem showed a steady increase in human immune status, particularly the antitumor status. This gives special relevance to the evaluation of the properties of the designated biosystem in order to further develop the corresponding fungo-therapeutic immunomodulatory and antitumor drug. According to the results of technological research: physico-chemical characteristics of Shiitake biomass dry powder (including crystallographic) were studied; the possibility of its tableting using only antifriction auxiliaries was substantiated; the optimum technology of tablet manufacturing with an operating name "Shitavit" was presented, an integrated approach to its creation was presented, the qualitative and quantitative composition of the drug was experimentally developed. The direct compression method was rejected due to unsatisfactory performance of all samples of tablets. The chosen technology of using preliminary granulation and the possibility of short-term contact of the L. edodes dry powder with a granulation liquid and an adjustable temperature regime allows all the properties of the biologically active substances of the designated raw material to be preserved. The research results for all series of the designated object showed relative stability of parameters for all indicators in laboratory samples. The information presented in this article is a guide to further research, necessary for a better understanding of the healing properties of fungal polysaccharides, increasing the use of broad-spectrum fungi-based preparations,leading to improvement of the quality of human life.
Вивчення якісних характеристик для вибору кардіологічних препаратів рослинного походження Мета: вивчення оцінки номенклатури лікарських засобів, що дозволяє виділити з усього ма-сиву лікарських препаратів рослинного походження (ЛП РП) ті лікарські засоби, що найчастіше призначаються лікарями, а також виявити номенклатуру малоефективних і небажаних для засто-сування ЛП РП.Матеріали і методи: відповідно до мети експертизи і можливостей проведеного дослідження нами був обраний метод очного анкетування, що дає можливість пояснити експертам суть питань, забезпечує отримання більше продуманих відповідей, які були використані нами для отримання достовірної інформації про номенклатуру ЛП РП і оцінки, а також про їх функціональні властивості і конкурентоспроможність.Результати дослідження. На думку більшості респондентів-лікарів (59,1 %), майбутнє фармако-терапії полягає в поєднаному застосуванні для терапії серцево-судинних захворювань (ССЗ) синте-тичних ЛП і препаратів рослинного походження. На думку фармацевтичних працівників, найбільш важливими властивостями ЛП РП, що впливають на призначення препаратів, є: вартість препарату (95,3 %), ефективність при застосуванні (80,5 %), країна-виробник (70,6 %), побічна дія (60,9 %), форма випуску і дозування (35,6 %), спосіб застосування (26,8 %). Установлено, що більшість ліка-рів-кардіологів (59 % опитаних) вважають за краще отримувати інформацію про ЛП РП із матеріа-лів науково-практичних конференцій, а також оглядів і наукових статей, що містять результати клінічних випробувань лікарських препаратів РП (63 % респондентів).Висновки. У ході аналізу досліджень установлено, що необхідно проводити профорієнтаційну роботу серед медичного персоналу для надання інформації про новітні лікарські препарати рос-линного походження і методи лікування, інформувати хворих щодо профілактики для попереджен-ня розвитку захворювань серцево-судинної системи.Ключові слова: кардіологічні препарати; серцево-судинні захворювання; анкетування; експертизаThe study of the quality characteristics for the choice of cardiological drugs of plant origin Aim. In accordance with the objectives of the expert evaluation, as well the possibilities of the research, the method of questioning was chosen. It allows explaining the essence of the issues to the experts, providing more thoughtful responses to obtain reliable information about the nomenclature of herbal drugs, their functional properties and competitiveness.Results.
