Resumen Introducción: La neoplasia intraepitelial corneal es una enfermedad displásica dentro del espectro de la neoplasia escamocelular de la superficie ocular. Es una enfermedad rara que se sospecha en pocas ocasiones, sobre todo en su presentación difusa. Una vez teniendo sospecha, el diagnóstico y l tratamiento son sencillos para una enfermedad que, no tratada, podría ser devastadora. Reporte de caso: Presentamos el caso de un paciente varón de 63 años de edad que presenta como único síntoma disminución de la agudeza visual con 2 semanas de evolución en el ojo derecho. La capacidad visual se mantiene en 20/40 en el ojo derecho y a la exploración mostró lesión geográfica en la mitad superior de la córnea derecha con apariencia en escarcha. Tras descarte diagnóstico se decide realizar un raspado epitelial con reporte de neoplasia intraepitelial corneal de bajo grado por parte del departamento de patología. Se decide manejo con mitomicina C al 0.02%, dos tantas de 2 semanas con tres aplicaciones tópi-cas por día, y fluorometolona al 0.1% en la misma dosis. Introduction: Corneal intraepithelial neoplasia is a dysplastic condition within the ocular surface squamous cell neoplasia spectrum. It is a rare condition that is seldom suspected, especially at its diffuse presentation. Once suspected, diagnosis and treatment are easy for a condition that if left untreated could be devastating. Clinical case: We present the case of a 63-year-old male patient presenting with a history of two weeks of right eye visual acuity decrease as only symptom. Right eye visual acuity was 20/40 and physical examination revealed a right cornea upper half geographic lesion with frosted appearance. After diagnostic rule out, epithelial scraping was decided, with low-grade corneal intraepithelial neoplasia reported by the pathology department. Management with mitomycin C 0.02% in two 2-week cycles with 3 topical applications per day, accompanied by fluorometholone 0.1% at the same dosage was then decided. It is a rare disease that is often subdiagnosed, especially when it presents in its diffuse form. When suspected, the diagnosis and treatment can be simple, even when we are dealing with a disease that can be disastrous. Case report: We present the case of a 63 year-old male patient that presents complaining of poor vision on the right eye with 2 weeks of evolution. Visual acuity maintained at 20/40 while corrected and at examination showed a superior corneal geographical lesion with frosted appearance. After other diseases were discarded, an epithelial scraping was performed and a low grade intraepithelial corneal neoplasia reported. Treatment with two series of 2 weeks of mitomycin C at 0.02% with three applications per day was decided accompanied with fluorometholone 0.1% at same dosage. Conclusions: The authors recommend the use of mytomicin C in countries in developing countries.
