Study results from early 2016 across 20 private general hospitals around Medan city, show there are 10 hospitals that have a physician performance measurement. However, the Medical Committee perceived some unknown barriers on measurement of physician performance. Should it be known, the hospitals can improve and enhance physician performance measurement effectively and efficiently. Therefore, this study aims to uncover the barriers on physician performance measurement in Private General Hospitals around Medan city. From the interviews with 10 Medical Committees and 6 document studies in 10 Private General Hospitals around Medan city which have a physician performance measurement, we found that the barriers on physician performance measurement are: unsupportive human resources, improper Medical Committee monitoring, unwillingness of physicians to be assessed, and assessors tendency toward giving moderate and good score. Therefore, we recommended that Medical Committee in each Private General Hospital around Medan or supervisors who will assess the performance of the physicians to attend performance measurement training to minimize biases and errors in filling out the sheet of physician performance measurement. Training should also be followed by an explanation that physicians should treat this performance measurement as a positive thing, because it can help the medical profession improve its professionalism.
The complexity of diversity, relationships, variety and specialization can provide more opportunities for mistakes, one of which is in the children's hospital services. Based on patient safety incident report data at Rumah Sakit Umum Haji Medan in January 2018 to October 2018, it is known that child care is the unit with the highest number of patient safety incidents compared to other units at 37 incidents. Implementation of patient safety by officers in children's services is the main focus that must be considered its role to prevent the occurrence of patient safety incidents. This research is a qualitative research that aims to see the extent of the implementation of patient safety in child care at Rumah Sakit Umum Haji Medan from the description of the role of health workers involved in child care, namely the role of the head of a child's SMF, the role of a pediatrician, and the role of a child nurse. Data collection was carried out by in-depth interviews with 7 informants and through observation. The results showed the implementation of patient safety in children's services has not been running optimally. This is because not all health workers in child services, namely the head of the child's SMF, pediatricians and child nurses do their part in the patient safety system. The roles carried out are still focused on the standards of each profession. It is expected that routine socialization on the implementation of patient safety, the implementation of special meetings and discussions to study the patient safety system and the existence of patient safety drivers in child care designated as the person responsible for moving every officer to implement patient safety. The high number of unmet needs affects the Total Fertility Rate (TFR) and birth spacing as well as the occurrence of unwanted pregnancies which also increases the risk of death in the mother. This study aims to analyze the unmet need for family planning in women of childbearing age in West Medan. This research is a quantitative study using a cross-sectional study design. The research sample was women of childbearing age in Karang Berombak Village, West Medan, Medan City. Samples were taken in 7 neighborhoods with a simple random method. Data obtained a sample of 162 people. The research data were analyzed using statistical software with logistic regression methods. The results showed that the number of unmet needs in Karang Berombak Village was 24.7%. Factors related to the incidence of unmet need, the decision to have KB is a decision by yourself / not involving the husband (Exp (B) 1.58 with a p-value of 0.02, after being controlled with the knowledge variable. The conclusion is that the number of unmet needs in Karang Village Wavy West Medan is still higher than the North Sumatra and the national rate. The factors that are associated with the incidence of unmet need are low levels of knowledge and the decision for family planning was the only women’s decision/not involving the husband. It is recommended to related agencies be more active in socializing about family planning and its side effects so that there is no misunderstanding about family planning.
