Las crisis epilépticas no provocadas representan un reto en la consulta de neurología. La identificación de la primera crisis no provocada debe ser el primer paso para el adecuado tratamiento. Para ello existen diferentes herramientas paraclínicas que ayudan al diagnóstico y establecen porcentajes de riesgo de segunda crisis o presencia de epilepsia, así como los factores de recurrencia de una crisis epiléptica no provocada de primera vez. Igualmente se abordan las indicaciones de tratamiento farmacológico para la primera crisis epiléptica no provocada y avanzamos hacia las indicaciones de referencia al siguiente nivel de atención médica. Un componente importante del manejo después de la primera crisis no provocada son las recomendaciones a pacientes o familia después de la primera crisis epiléptica de primera vez, que de igual manera se aborda en esta sección. Esta guía mexicana se elaboró tomando en cuenta los recursos de nuestros hospitales para herramientas diagnósticas en medio público y privado.
Unprovoked seizures represent a challenge in the neurological clinical consult. Identifying a first unprovoked seizure is the first step for an adequate medical approach, for which there are different diagnostic tools that help establish the risk of a second seizure, as well as recurrence factors for a first unprovoked epileptic seizure (UES) or diagnose epilepsy. Pharmacological treatment for a first UES and key points for referral to specialists are similarly established, and we move forward to the reference for the next level of medical attention. In this section we also describe nonmedical recommendation for patients and family members after a first UES. This Mexican Guideline was elaborated accounting for the resources and diagnostic tools available in both public and private hospitals in Mexico.
