BackgroundSexual expression by forensic psychiatric patients is poorly researched.MethodsForensic experts representing 14 European countries were interviewed to explore the diverse ways in which sexual expression within forensic settings is handled.ResultsNo country had a national policy, although many had local policies or shared practices. Progressive approaches to patient sexuality were evident in nine of the countries sampled. Purpose
Investigating patients’ perceptions of their illness can provide important insights into the experience and management of the illness and associated treatment, and enhance understanding of variations in adherence to prescribed medication. The Common-Sense Model of Self-Regulation (CSM) provides a theoretical framework for the study of illness cognitions, health behavior, and adherence to health recommendations. The aim of this study was to use the CSM to investigate the experience of glaucoma and its treatment from the patients’ perspective, and to apply these insights to classify and clarify issues related to nonadherence with treatment.
Patients and methods
A qualitative investigation using semi-structured interviews took place in two outpatient glaucoma clinics. Thirty-three patients with primary open-angle glaucoma using hypotensive eye drops participated in the study. Deductive content analysis was used to analyze the interview data.
Results
Issues relating to nonadherence with hypotensive eye drops and patients’ experience with their glaucoma and treatment were identified. Treatment schedule and patient factors were classified as common barriers to adherence. Further themes include experienced symptoms of glaucoma, illness coherence, and the emotional and practical consequences of the illness.
Conclusion
Findings provide important insights into the emotional and practical outcomes of glaucoma for patients, perceived symptoms of the illness, and insights into patient memory and cognition. These findings provide supporting evidence for the importance of conducting theoretically driven qualitative investigations of patients’ experience with glaucoma and their treatment, and provide suggestions on key issues that need to be addressed in future multidimensional interventions aimed at improving adherence and patient quality of life.
Research in the field of narrative psychology has found that redemption -a narrative sequence in which people recount emotionally negative experiences as having positive endings -is a useful mechanism for coping with adversity. Redemption has been viewed as a cultural master narrative in North America, providing individuals with a socially valued script for narrating challenging life experiences. Presently little research has examined the presence and function of the redemption narrative outside of North American contexts. The aim of this qualitative study, therefore, was to identify themes in UK individuals' narratives of trauma and adversity to gain insight into the content of the master narrative for meaning-making in the UK. Sixty-Five participants (57 females, Mage = 21.97, SD = 7.24) with little to no experience of lifetime adversity were recruited into an online survey. Participants answered open-ended questions adopting the perspective of a survivor from a selected UK national tragedy, focusing on how they felt survivors could recover from trauma. We identified two themes in our thematic analysis that were relevant to recovery: recuperation and redemption. Recuperation was most commonly reported, it was described as a gradual lessening of symptoms over time and the ability to cope with the lasting emotional and physical scars. Our findings suggest that redemption is not necessarily the dominant cultural script in the UK for guiding recovery in the aftermath of trauma.
