Geriatric population in India is rapidly increasing. According to one estimate the number of persons above the age of 60 years is 76.6 million in India. India as the second most populous country in the world, with elderly people constituting 7.7% of total population. The morbidity in this age group is often due to increased incidence of falls. It is often preceded with spells of dizziness and often associated with vestibular disorders. Peripheral vestibular disorders often difficult to evaluate at the time of fall and only realized after the fall. Peripheral vestibular disorders like Benign Paroxysmal Positional Vertigo (BPPV), Vestibulopathy, Vestibular Neuritis are some of the disorders that affect geriatric population that affect the functional ability in 'activities of daily living (ADL). BPPV has been frequently found to be more common with advancing age. One simple test to assess ADL (Activities of Daily Living) is TUG test (Timed Up and Go). A score of 11.1 seconds or more on TUG test was correlated with 80% chance of a fall in the elderly. Hence a study is undertaken to evaluate peripheral vestibular disorders in elderly with special reference to BPPV along with their performance on TUG (Timed Up and Go) test. Benign Paroxysmal Positional Vertigo (BPPV) has been found to be most common vestibular disorder in the elderly. One out of four elderly with Peripheral vestibular disorder has a risk of 'fall'. BPPV is the most common peripheral vestibular disorder leading to 'risk of fall' three times more among the elderly after 75years.
Ketogenic Therapy (KT) is a treatment for intractable epilepsy shown to be effective in reducing seizures. This diet consists of high fat, adequate protein, and low carbohydrate. Although KT has been used for decades, the potential adverse effects are unknown. To evaluate the potential for growth abnormalities, a retrospective analysis of patients since 1995 and on KT for at least 1 year was performed. Data was collected in an Access database and a macro was used to determine the BMI and z scores. Statistical analysis was done with significance reported by p values <0.05. Ht z score became progressively worse over time. The most significant difference from 0–6 mo was found at >60 mo on diet. The only significant change from baseline for wt z score was seen at >60 mo on diet (μ=−0.9±1.8, −1.6±1.4, p=0.036). There were no significant differences compared to baseline for BMI z score. The question of decreased linear growth being a direct adverse effect of KT remains unknown. Some evidence has shown that epileptic patients have lower stature compared to a healthy, normal population. Additional controlled, prospective studies should be designed to further investigate the effect of KT on growth.
<p class="abstract">Basal cell carcinoma (BCC) of nose is common with potential of local recurrence and high risk features. We are reporting a case of BCC on the right ala of nose in an elderly female patient hailing from rural background, who came to our OPD for cosmetic outcome. She was treated with wide local excision with nasolabial flap reconstruction. The tissue when sent for HPE came up with BCC with margin involvement. Hence she was sent to chemoradiotherapy after 3 weeks of surgery, after complete wound healing.</p><p class="abstract"> </p>
