Between 1980 and 1985, 147 patients with lumbar disc herniations were treated by discectomy and/or foraminotomy.,We investigated preoperative clinical and myelographic findings. Lasegue's sign was positive in 50% of cases.It was positive more frequency in younger patients.Diagnosis of the clinical level of L % disc herniation was more accurate than of L, S, disc herniation.The average sagittal diameter of all dural tubes was 13. 8mm and that of the dural tubes with complete block was 11. 6mm.Ventral defect of L 44 disc herniation was larger than that of L 5, S 1 disc herniation.
We repart a case of osteochondral fracture of the talus with rupture of the anterior talo-fibular and calcaneo-fibular ligaments, treated by using an absorbed pin. The subject was a 30 year-old man who was injured while playing tennis.Plain X-ray showed a partially detached fragment which we diagnosed as a Berndt classification stage 2. Three days after the injury we operated by using 2 absorbed pins and ruptured ligaments. AT the time of surgery the fragment was found to be completely detached, so we revised the diagnosis to a Berndt stage 3.The patient was immobilized in a cast for 4 weeks after which he started active excise of the angle. Weight bearing of 10kg was allowed form the 6th week after surgery and increased by 10kg each week. Good results were achieved using this treatment.
This paper reports a rare case of fracture-dislocation of the hamatometacarpal joint. A 25-year -old right-handed man was involved in a fistfight and struck a wall with his left fist . He was examined at our hospital 5 days after injury and found to have swelling and tenderness in the vicinity of the fourth and fifth CM joints. Radiographs of the hand revealed dorsal dislocations of the bases of the fourth and fifth metacarpal accompanied by a dorsal oblique intra-articular fracture of the distal hamate and an avulsion fracture of the base of the fourth metacarpal. Treatment consisted of closed reduction and percutaneous fixation with Kirschner wires. Three years after his injury, the patient is asymptomatic, working at his previous job, with a full ROM of all joints of his left hand wrist and normal grip strength.-131-
