Objectives This study aimed to assess and compare the effectiveness of three different remineralizing agents (Tricalcium phosphate paste, Fluoride varnish, and Nano-hydroxyapatite gel) using the DIAGNOdent device. Material and Methods The present clinical study was carried out on 90 initial carious lesions detected by ICDAS caries diagnostic criteria and then take the baseline record by DIAGNOdent device. The selected initial carious lesions were randomly classified into three groups according to treatment modalities (30 lesions in each group) according to remineralizing agents: group A (TCP), group B (fluoride varnish) and group C (nano-hydroxyapatite gel). The remineralizing agents were applied for four minutes once weekly for four weeks. At the fifth week, the DIAGNOdent scores of initial carious lesions were recorded to evaluate the effect of remineralizing agents. A paired t-test was used to compare between baseline date and follow up of DIAGNOdent scores. A one-way ANOVA test was used to compare DIAGNOdent scores among the three groups. Post- Hoc Tukey test was used to determine the significant difference between every two groups. Results There were statistically significant differences among the three groups at follow up (p = 0.001). Within each group, there was a significant difference between baseline and follow up scores (p = 0.000 for the three groups). Multiple comparisons between every two groups showed a highly statistically significant difference at follow up records between nano-hydroxyapatite versus TCP and fluoride varnish on pit and fissure caries (p = 0.039 and p = 0.007 respectively) and the nano-hydroxyapatite was the best of them. Conclusion The present study concluded that the three remineralizing agents were effective in the treatment of initial carious lesion and the most effective remineralizing agent was nano-hydroxyapatite.
Objectives: This clinical trial evaluates the efficacy of different pit and fissure sealant modalities as a caries prevention tool. Materials and Methods: Forty-four children aged 6–8 years with completely erupted first permanent molars participated in sealant placement randomized clinical trial. Split-mouth technique used and each child received four treatment modalities (ICON, Seal It, GCP glass seal, ICON/Seal It combination). Caries progression and sealant retention monitored over a period of 24 months. Results: Seal It and combination modalities had the highest efficacy in the prevention of pits and fissures caries, whereas the ICON had the lowest efficacy. Regarding retention, there were no significant differences among the four modalities during the 1, 3, and 6 months follow-up intervals. However, at 12, 18, and 24 months, the retention of Seal It and combination modalities were better than ICON and better than GCP, but the differences were not statistically significant. In addition, the retention of combination modality was better than Seal It, but the difference was not significant. At the end of the trial, the caries incidence was the highest in ICON group (20 lesions) followed by GCP group (13 lesions), Seal It group (3 lesions) and the lowest observed in combination group (2 lesions). Conclusions: Both Seal It and combination modalities are highly effective in the prevention of pits and fissures caries, GCP has acceptable effect while the use of ICON alone was not effective. The Seal It and ICON combination improves their clinical efficacy in caries prevention.
A BSTRACT Background: White spot lesions (WSLs) are considered as the first sign of the progression of dental caries that can be reversed. Casein phosphopeptide–amorphous calcium fluoride phosphate (CPP-ACFP) varnish can promote remineralization process of the demineralized tooth structure. Also, WSLs can be managed by using caries infiltrant (ICON), which infiltrates etched enamel surface of the WSLs. Objectives: The main aim was to evaluate and compare the efficacy of ICON versus CPP-ACFP varnish to treat WSLs in the same oral environments. Materials and Methods: A total of 30 participants aged 20–40 years with at least two WSLs have participated in this trial. Split-mouth technique has been used in this trial where WSLs on the left side have been treated with ICON, whereas WSLs on the right side have been treated with CPP-ACFP varnish. Clinical evaluation using DIAGNOdent was carried out at baseline, 1 week, 1, 3, 6, and 12 months after application to determine the effectiveness of these materials in treating WSLs. Results: Significant differences were observed between all follow-up DIAGNOdent scores compared to baseline readings of both ICON and CPP-ACFP ( P = 0.000 for all comparisons). Statistically significant differences were observed between DIAGNOdent scores of ICON compared to CPP-ACFP at all follow-up intervals ( P = 0.006, 0.008, 0.045, 0.036, and 0.000). Conclusion: Both ICON and CPP-ACFP were effective in the treatment of WSLs of smooth surfaces but the efficacy of CPP-ACFP was better than ICON.
