The World Health Organization declared the problems of dorsopathies (vertebrogenic syndromes) as priorities for the next five-year period. The key questions of etiology and pathogenesis of dorsopathies for the last period has been revised, which affects the principles of treatment and rehabilitation of this category of patients. This article highlights the main clinical and pathogenetic characteristics of the process. A diagnostic algorithm, mechanisms of action and therapeutic possibilities of the main methods of correction are considered. Also the article considers the principles of medical treatment and physical stimulation, with the confirmation of the fea-sibility of an integrated approach that provides effective relief of manifestations of dorsopathies. In this case, taking into account authors’ original research, attention is paid to theoretical and practical aspects of acupuncture, prescribed in the treatment of patients with dorsopathies. The authors described an effective technology pharmacopuncture, an action of which is caused by the summation of the effects of reflex irritation, actions of the drug. Based on the results of many years of personal research, the authors note that pharmacopuncture involves the use of various drugs, but just the complex homeopathic remedies provide a clear therapeutic result.
Dorsopathies are enduring importance among medical problems. This confirms the fact that the World Health Organization declared a theme associated with dorsopathies, as priorities for the next five-year period. It should be emphasized that the etiology and pathogenesis of these states are reviewed with the «enviable» frequency, and this, naturally, is reflected on the principles of treatment and rehabilitation of this contingent. In this review the mechanisms and features of application of the basic drug and non-drug methods of treatment used in modern clinic, are considered. It should be noted that a use of a complex approach, aimed at correction and relief of various vertebral and pathological manifestations, is optimal in this case. In particular, the review presents in details the options for medical treatment and a variety of technologies of physical stimulation - from traction to modern hardware methods of physiotherapy. Taking into account authors’ own original developments, special attention is paid to theoretical and practical aspects of reflexology, which is widely used in complex therapy of patients with dorsopathies. In this block, the authors provide Oriental, traditional approaches to acupuncture and promising ways, based on the combined use of Eastern and Western medical schemes. The authors identify separately the pharmacopuncture as the action caused by the summation of the effects as reflex irritation and entered in the field point of medicaments. Pharmacopuncture involves the use of various medicines, but complex homeopathic remedies provide a distinct therapeutic result, i.e., the authors suggest that there are subtle mechanisms of interaction between point and drugs of this class.
Over the last 15 years 40 dissertations on traditional medicine, in particular – on reflexology, were presented in the Dissertation Council at the Russian Scientific Center “Medical rehabilitation and balneology". Among them, 11 works were on competition of a scientific degree of the doctor of Sciences and 31 – scientific degree of the candidate of Sciences. Thesis presentation in chronological order is as follows: in 2001 – 4 works; in 2003 – 7 ; in 2004 – 6; in 2005 – 3; in 2006 – 2; in 2007 – 1; in 2008 – 6; in 2009 – 5; in 2010 – 1; in 2011 – 3; in 2012 – 1; in 2013 – 1 and, finally, in 2014 – 2. Thus, there has been some reduction in the number of works, and, possibly, the decline of interest in the problem itself. With a certain degree of conventionality, the authors have divided the thesis into blocks "Fundamental research", "New technologies", "Traditional medicine in diseases of the nervous system and internistiche pathology", etc. It should be recognized that all dissertations were defended, but are not of equal quality. The authors note that in each block there are works devoted to the important issue - the combina-tion of acupuncture and homeopathy, including pharmacopuncture. The presented studies were discussed adequately, emitting positions "contingent", "methods", "general provisions".
Due to the active professional activities of women aged 45-60, in the modern society, the after-care treatment of female patients with essential arterial hypertension (stages I-II , grades 1-2 ) under pressure of climacteric syndrome is of high social and economic importance. The specialists of medical rehabilitation are the problem of finding effective technologies of medical and non-medical series for improving the quality of life of women in this age group. This article presents the results of after-care treatment of women with essential arterial hypertension and mild or moderate climacteric syndrome (92 females aged 50±4.5) by means of the use of standard hypotensive pharmaceuticals (Candesartan, Bisoprolol, and Hydrochlorothiazide), non-hormonal pharmaceuticals (Klimadynon Uno), and spectral phototherapy as a physiotherapy approach. Such treatment resulted in reliable stabilization of blood pressure, vegetative functions, responsiveness, anxiety, encephalitic circulatory dynamics according to the REG data, and WAN indicators. The average clinical response of the control group amounts to 81.4 %, while that of the experimental group makes up 65.5 %. The key results are sustainable blood pressure stabilization, improved overall health, decreased frequency and severity of the climacteric syndrome along with stabilized vegetative functions, improved encephalitic circulatory dynamics, and psycho-emotional state.
The article presents the comparative analysis of the effectiveness of various technologies of acupuncture in treatment of patients with vertebrogenic syndromes. The study included 90 patients at the age of 52 years with a diagnosis of lumbo-sacral dorsopathy; methods of examination of this group were clinical, pathopsychological and instrumental, including analysis of somatosensory evoked potentials. The correction methods were intramuscular injection of homeopathic drug Target T and pharmacopuncture as this tool, and saline solution. The authors indicate that the results observed in the course of pharmacopuncture, are not only reflex and medical components. However, the healing mechanisms of the technology didn’t fully disclosed; the researchers deliberately avoid use simulation pharmacopuncture (placebo) in dorsopathy, indicating the complexity of its implementation and an a priori lack of effectiveness. The present work is aimed at solving this problem by extending the evidence base of reflexology. The study found that the rate of formation of positive reactions and their stability are directly dependent on the applied reflex-technologies. In particular, the reflex potentiation and drug effects in the course of pharmacopuncture with the use of the drug Target T, and the volatility of results in response to the use of saline solution were identified. In addition, delayed, cumulative therapeutic effects of the standard use of the drugs were presented. It was confirmed by follow-up data. The solution of the concrete problem - the possibility of using placebo in studies on acupuncture - has been achieved.
