One of the most relevant and promising areas in the world of oncogynecology, enabling deeper understanding of the etiology and pathogenesis of malignant growth, to study the processes occurring in women, is the study of molecular biological markers, characterizing apoptosis, cell proliferation and angiogenesis. To increase the effectiveness of combined treatment of the patients with endometrial cancer I−II (Т1−2N0M0) stages and prevent the occurrence of local recurrences as well as distant metastases on the basis of investigating the molecular biological features of tumor and their prognostic value, a study based on clinical observations of 125 women was performed. Their treatment included surgery and a postoperative course of remote radiation therapy. The strategy of adjuvant therapy was designed according to the definition of the group of increased risk of cancer recurrence in the patients with negative receptor status (ER, PR) based on the degree of differentiation of endometrial adenocarcinoma (G), depth of tumor invasion into the myometrium and its location in the cavity as well as the level of expression of molecular biological markers (VEGF, Bcl−2, Ki−67). To select the tactics of further treatment, each of the indices was evaluated in points on a scale. Next, the intervals of the total value of S points were set, depending on which the program of combined adjuvant therapy was chosen: up to 6 points the contact radiation therapy was performed, 7−12 points − there was applied a remote in the postoperative period; 13 points − after remote and contact radiation, six more cycles of chemotherapy were added. Analysis of the research results showed that the developed method of combined treatment of patients with endometrial cancer I−II (Т1−2N0M0) stages allows the personalization of the protocol of adjuvant therapy and recurrence prevention. Background. Radiation therapy is an essential in the complex of modern cancer care methods, since it is needed by more than half of cancer patients worldwide. However, each year more than 2 million people from low-income countries are unable to access modern radiotherapy technologies [1]. Thus, the urgent task in the field of radiation oncology is to increase the availability, quality and efficiency of radiotherapy, monitoring the level of technical, methodological and human resourcing of radiation therapy in Ukraine in accordance with trends in Europe and the world. The actual task of the study is to review and analyze the existing level, availability and effectiveness of applying the modern technologies of radiation therapy in the regions of Ukraine. Purpose – analyzing the current state and level of availability of radiotherapy in Ukraine, seeking for opportunities to expand applying it. Materials and methods. Analyzing and comparing statistical data of the National Cancer Registry of Ukraine and the information of IARC GLOBOCAN, IAEA DIRAC Database, summarizing the results. Results. The level of technical and technological equipment as well as staffing of oncology centers of Ukraine has been analyzed, along with assessing quantitative and qualitative components of radiotherapy care in comparison with the average European indicators. The assessment of the state of radiotherapy care in Ukraine was carried out with due regard for the increasing number of patients. Conclusions. According to the WHO, the structure of cancer incidence in Ukraine is similar to the European one, however higher specific mortality rate indicates, first of all, the insufficient level of covering the population of Ukraine by specialized diagnosis and oncological care, its low efficiency and engineering backwardness. It has been determined that radiation therapy in Ukraine is needed annually by at least 90 thousand cancer patients. In fact, only about 44 thousand of them are provided with tele-radiation therapy. First of all, this is due to extremely insufficient radiotherapy equipment available in Ukraine and using outdated radiation treatment technologies in gamma-ray teletherapy that result in its low quality and aggravating the clinical consequences. Given the growing number of cancer patients in 2022, Ukraine will need about 160 machines for tele-radiotherapy. The rise and adjusting the clinical and technical capabilities of regional oncology centers in terms of quality and efficiency of radiation therapy is possible under the conditions of their rapid technical.
Relevance. Endometrial cancer ranks first among oncogynecological diseases, however, 5-year survival can be achieved only in 67,7 % of patients, while 22,4 % of patients die during this period from the continuation of the tumor process. Therefore, improving the complex therapy of patients with EC is very important. Purpose of the study. to investigate therapeutic pathomorphosis in patients with advanced endometrial cancer with neoadjuvantchemoradiation therapy. Materials and methods. In order to study radiation pathomorphosis, 26 patients with ER T1-3N0-1M0-1 were examined. Group I consisted of 11 patients with a preoperative course of external radiation therapy SOD 30 Gywithout radiomodification, II – 15 patients with a preoperative course of external radiation therapy SOD 30 Gywith radiomodification. Results and its discussion. When conducting a comparative assessment of radiation pathomorphism in patients of the study groups, it was found that when using the preoperative course of TFD on Gy 30 with radiomodification, a significant decrease in the mitotic index occurs. There is also a tendency to an increase in the degree of tumor regression, the number of pathological mitoses and stromal reactions during the preoperative course of TFD on 30 Gywith radiomodification. Conclusions. It was found that radiation therapy with EС with radiomodification gives a more positive effect and regression of more than 50,0 % of the tumor volume, a decrease in its biological potential. The volume of the damaged tumor was 58,0 %, and the volume of the residual tumor was 42,0 %, that is, most of the tumor mass underwent regression.
Objective: to investigate the problem of greening economic relations in the management of economic processes to determine the essential content of this category, to outline the main problems and identify existing obstacles to its greening of economic relations in the management of economic processes, as well as to try to develop an economic management mechanism. on the financial interests of the participants involved in the processes related to the use of natural resources. Method. The research led to the use of methods of scientific knowledge: formal logic, analysis and synthesis, systems approach, theoretical generalization, causation, scientific assumption, comparison, expert research methods. Results. The study showed that in defining the essence of the category of "greening" in scientific circles has not yet formed a single approach; scientists hold different views on this. It is established that quite a few of them understand the issues of integration of ecological and economic aspects of greening. The article defends the position on the need for such an integration combination. The interaction and relationships of participants in the process of greening, which the authors include society as a whole, the state and enterprises that use natural resources in their economic activities. The inclusion of economic incentives in the mechanism of environmental management, which is based on the economic interests of these entities, is justified. The main legislative acts regulating economic relations in the management system of economic processes taking into account the requirements of environmental nature are analyzed, a comparative analysis of the main provisions and identified imperfections of institutional tools for greening economic relations in the management system of economic processes in Ukraine. The author's definition of the concept of "greening" as a complex phenomenon formed on the basis of a combination of systems of processes and measures for the effective from the standpoint of economic and environmental criteria for the use of natural resources. The order of combination of economic interests in the system of greening of management of economic processes is specified. Scientific novelty. The essence of greening is defined as a system of processes and measures for the management and use of natural resources that meet the criteria of efficiency and savings, requirements for reducing the destructive impact on society and the environment, and are based on economic interests of all economic actors participation in the implementation of these processes and measures. The interaction of these economic entities is studied and the necessity of reforming the fiscal levers of influence on the users of natural resources is substantiated. Practical significance. The study proved the need to clarify the content of economic relations in management processes and increase the environmental and economic efficiency of the interests of business entities in modern conditions. The presented scheme creates preconditions for development of the effective mechanism of restoration of natural resource potential and increase of efficiency of managing. Endometrial cancer (EC) ranks second in the structure of oncogynecological diseases both in Ukraine and worldwide. More than 80 % of recurrences occur in the first two years after surgical treatment. Molecular biological markers are extensively studied in different oncological diseases, because they allow to understand the etiology and pathogenesis of malignant growth more deeply. Aim. To study molecular biological markers and tumor hormonal status in patients with EC and determine their practical significance. Materials and methods. Molecular biologic markers of the tumor were studied in 89 stage I–IV EC patients. To study the hormonal status of the tumor in EC patients, 119 examinations were carried out. Results. Significant correlations were found between the level of molecular biological marker (MBM) expression, hormonal status of the tumor, the presence of disease recurrence, and the age of EC patients. The novelty of the study lay in the substantiation of treatment individualization for EC patients based on the determination of the tumor molecular biological characteristics and their prognostic value. It has been proven that the expression level of the studied MBM, ER and PR receptors in EC patients differed depending on age, presence, localization and severity of disease recurrence. A group of patients with a high risk of the disease recurrence has been revealed among EC patients. For the first time, a technology has been developed for the combined treatment of patients with I–II (T1-2N0M0) stages of EC with a negative receptor status (EP–, PR–) and a high risk of the disease recurrence to assess MBM levels (VEGF, Bcl-2, Ki-67), degree of differentiation, myometrial invasion depth and tumor localization in the uterine cavity allowing to individualize the choice of adjuvant therapy regimen, thus improving the effectiveness of treatment and preventing the occurrence of locoregional relapses and distant metastases. Conclusions. For the first time, the expression levels of the studied molecular biological markers and ER and PR receptors have been shown to differ depending on age, localization, and relapse severity, which could be used to determine the risk of relapse in EC patients.
