New functional neuroimaging techniques are changing our understanding of the human brain, and there is now convincing evidence to move away from the classic and clinical static concepts of functional topography. In a modern neurocognitive view, functions are thought to be represented in dynamic large-scale networks. The authors review the current (limited) role of functional MR imaging in brain surgery and the possibilities of new functional MR imaging techniques for research and neurosurgical practice. A critique of current clinical gold standard techniques (electrocortical stimulation and the Wada test) is given.

One of the most important factors controlling material specific processing in the human brain is language dominance, i.e. hemispheric specialization in semantic processes. Although previous studies have shown that lateralized long-term memory processes in the medial temporal lobes are modified in subjects with atypical (right) language dominance, the effect of language dominance on the neural basis of working memory (WM) has remained unknown. Here, we used functional MRI (fMRI) to study the impact of language dominance on the neural representation of WM. We conducted an nback task in three different load conditions and with both verbal and nonverbal (spatial) material in matched groups of left and right language dominant subjects. This approach allowed us to investigate regions showing significant interactions between language dominance and material. Overall, right dominant subjects showed an increased inter-individual variability of WM-related activations. Verbal WM involved more pronounced activation of the left fusiform cortex in left dominant subjects and of the right inferior parietal lobule in the right dominant group. Spatial WM, on the other hand, induced activation of right hemispheric regions in left dominant subjects, but no specific activations in right dominant subjects. Taken together, these findings indicate that the neural basis of verbal WM processes depends on language dominance and is more mutable in right dominant subjects. The increased variability in right dominant subjects strongly suggests that a standard network of material-dependent WM processes exists in left dominant subjects, and that right dominant subjects use variable alternative networks. Кемерово, Россия до настоящего времени в медицинской практике реабилитация пациентов с афазией, перенесших ишемический инсульт, заключается в логопедических методиках, направленных на восстановление артикуляционного аппарата, в соответствии с феде-ральной программой помощи сосудистым больным, тогда как патогенез речевых нарушений при афазии в большинстве случаев значительно сложнее, чем простая дисфункция артикуляции, и связан с нарушением речемыслительной деятельности в целом и, как следствие, нарушением номинативной функции языка.Антропоцентрическая парадигма изучения языка, пришедшая на смену системоцентрической, направлена на исследова-ние языковых явлений с когнитивных позиций. Когнитивная лингвистика помогает не только понять механизм речемыслительной и рече порождающей деятельности человека, но дает возможность воздействия на звенья этих процессов, в частности при восста-новлении речи у больных с афазией.Предлагаемая концепция речевой ресоциализации направлена на реабилитацию пациентов, перенесших ишемический ин-сульт, за счет функционального возбуждения новых участков мозга, не задействованных до заболевания, путем активации непо-раженных отделов мозга при морфологическом повреждении зон Брока и Вернике в результате инсульта. Применение лингвопер-сонологических и социолингвистических факторов реабилитации (профессия, увлечения, уровень образования, гендерный фактор и пр.) позволяет индивидуализировать разрабатываемую универсальную методику в каждом конкретном случае.Оценка эффективности реабилитации при афазии возможна с использованием международных шкал NIHSS и MMSE, вклю-чающих в себя элементы нейропсихологического обследования, а также функциональной магнитно-резонансной томографии (фМРт) на базе последовательности BoLd.Ключевые слова: динамическая афазия, пропозициональная структура, пропозиция, производное слово, словообразователь-ный тип, словообразовательная ниша, гнездо однокоренных слов, полисемия, синонимия производного слова, полимотивация, фМРт.
