Construct Validity and Physical Performance of Older Adults in Different Hierarchical Physical-Disability Levels

Abstract: The purpose of this study was to test the construct validity of the hierarchical levels of self-reported physical disability using health-related variables and physical-performance tests as criteria. The study participants were a community-based sample of 368 adults age 60 years or older. These older adults were grouped into 4 levels according to their physical-disability status (able, mildly disabled, moderately disabled, and severely disabled groups) based on their self-reported measures on the mobility, ins… Show more

“…A powerful association between ADL disability and mortality is recognized (Keeler et al, 2010). Thus our results support other studies demonstrating a hierarchical pattern of functional decline starting with limitations in mobility progressing on to disability in ADL and subsequent mortality (Guralnik et al, 2000;Wang, Sheu, & Protas, 2007). It is further recognized that walking requires energy and puts demand on multiple organ systems .…”

“…This composite measure, which captures a broad range of disability from early or "preclinical" disability to later personal care disability, has the advantage of capturing finer graduations in limitations and reducing ceiling or flooring effects. 17 Cronbach's alpha of the 16 items across all waves averaged 0.90.…”

“…26,27 Test-retest reliability is high for both scales, ranging from 87% to 100%. 28 Using disability data from both scales, we created a dichotomized outcome of respondents with a disability 28 years later (people who encountered some degree of difficulty performing at least one of the basic or instrumental activities of daily living) versus respondents without a disability 28 years later.…”
