Lipid peroxidation induced by reactiveoxygen species (ROS) is involved in the pathogenesis of malignancy. Overall, lipid peroxidation levels are indicated by malondialdehyde (MDA), which is the most frequently used biomarker to detect oxidative changes. Antioxidant defense systems such as glutathione (GSH) limit cell injury induced by ROS. Therefore, MDA and GSH can be used to monitor oxidative stress (OS). Hence, this study aimed to evaluate and compare both salivary and serum levels of MDA and GSH in oral leukoplakia and oral squamous cell carcinoma (OSCC) patients, and healthy controls. The study included 100 subjects comprising 30 apparently healthy controls, 30 patients with oral leukoplakia and 40 clinically and histologically diagnosed patients with OSCC. Saliva and blood samples were obtained and evaluated for MDA and GSH. The study revealed enhanced MDA levels in saliva and serum in oral leukoplakia and OSCC patients as compared to controls. On the other hand, significant decreases were seen in serum and salivary GSH levels in oral leukoplakia and OSCC patients as compared to controls. Augmentation of OS in blood and saliva is reflected by increase in MDA and decrease in GSH levels, indicating that tumor processes cause an imbalance of oxidant-antioxidant status in cell structures.(J Oral Sci 56, 135-142, 2014)

Introdução: O malondialdeído (MDA) é um biomarcador importante na detecção do estresse oxidativo em amostras biológicas. Objetivo: Analisar a associação entre os níveis séricos de marcadores de estresse oxidativo, o estadiamento e terapia no câncer colorretal e compará-los com indivíduos saudáveis. Método: A amostra foi composta por 40 pacientes com câncer colorretal, recebendo radioterapia e realizando ou não quimioterapia, e 80 indivíduos saudáveis, recrutados em uma clínica especializada em Oncologia, no período de janeiro a junho de 2013. O estresse oxidativo foi analisado através da peroxida..o lipídica, que foi baseada nas espécies reativas ao ácido tiobarbitúrico (TBARS). Os resultados foram calculados de acordo com curva-padrão feita com malondialdeído. O estadiamento do câncer colorretal foi obtido dos prontuários dos pacientes. As análises estatísticas foram realizadas por meio dos testes exato de Fisher e Mann-Whitney, sendo o valor de p<0,05 definido como estatisticamente significante. Resultados: A concentração sérica de MDA (μM/L) foi de 4,1±2,9 no grupo com cancer e de 1,1±0,4 no grupo sem câncer (p=0,001). Em relação ao estadiamento do tumor, a média de MDA foi de 2,3±1,5 entre o grupo com estadiamento inicial (II) e de 5,4±3,0 entre o grupo com estadiamento avançado (III) (p<0,001). Não houve associação entre concentrações séricas de MDA e terapia realizada (p>0,05). Conclusão: As concentra..es séricas de malondialdeído foram superiores no grupo câncer em relação ao grupo sem câncer. Além disso, os níveis elevados de malondialdeído foram associados positivamente ao grau de estadiamento do tumor.

Cisplatin is a chemotherapeutic agent widely used in the treatment of several solid tumours. For patients with advanced head and neck squamous cell carcinoma in whom surgery is contraindicated, treatment with high-dose cisplatin administered every 21 days for 3 cycles concomitantly with conventional radiotherapy is recommended [1][2][3].Its anticancer mechanism is mediated by DNA binding, which leads to the formation of inter-and intrastrand crosslinks and results in defective DNA templates, arrest of DNA synthesis and replication, DNA damage and, finally, cell death [4]. In addition, cisplatin accumulates in human cells (normal and tumour cells) resulting in enhanced production of reactive oxygen species (ROS) and reactive nitrogen species (RNS), which leads to mitochondrial damage and dysfunction [5,6]. In addition, there is a decrease in the antioxidant defence system mediated primarily by links formed between cisplatin and glutathione, which culminates in the depletion of glutathione [7]. The excessive generation of ROS and RNS damages biomolecules, resulting in lipid, protein and DNA/RNA oxidation and causing toxicities including nephrotoxicity, neurotoxicity, ototoxicity and hepatotoxicity [8][9][10][11]. The intensity of biomolecular damage can be determined by oxidative/nitrosative stress biomarkers. In this study, we used malondialdehyde (MDA) and lipid hydroperoxides, which are indicators of lipid peroxidation/ oxidative stress biomarkers and nitrite, a nitrosative stress biomarker.Therefore, the aim of this study was to determine the effects of high-dose cisplatin chemotherapy and conventional radiotherapy on urinary levels of MDA, lipid hydroperoxides and nitrite biomarkers in patients with head and neck cancer. Material and MethodsThis was a prospective study conducted from January 2013 to November 2013 at the oncology clinic of a public teaching hospital in the state of São Paulo, Brazil. The research ethics committee of the institution approved the study, and all patients signed a consent form authorizing the use of their data.Patients and treatment characteristics. All patients were diagnosed with primary squamous cell carcinoma of the head and neck by a biopsy and were administered antineoplastic treatment with high-dose cisplatin (80 or 100 mg/m 2 ) with concomitant conventional radiotherapy as a therapeutic regimen. Patients who had undergone previous treatments for their tumours (surgery, chemotherapy and radiotherapy), or who refused to participate in the study, were excluded. Patients were dropped from the study if their treatment regimen changed before the study commenced, if they died or abandoned the treatment, or if complete oxidative stress data were not available (biomarkers or measurement times).The treatment regimen consisted of 3 cycles of chemotherapy with high-dose cisplatin (80 or 100 mg/m 2 ) on days 1, 22 and 43.
