Prognostische Faktoren für den Erfolg einer intrauterinen Insemination: Eine Evaluation von 1005 Zyklen bei 349 Frauen

Abstract

Die intrauterine Insemination (IUI) stellt im Vergleich zur extrakorporalen Befruchtung eine sehr viel weniger invasive, weniger zeitintensive und nicht zuletzt kostengünstigere Methode der Sterilitätstherapie dar. Um den Stellenwert der IUI besser diskutieren zu können, wurde anhand von 1005 IUI-Zyklen bei 349 Frauen retrospektiv Prognosefaktoren analysiert. Bei 29,2 % der Patientinnen konnten mit einer Insemination eine Schwangerschaft erzielt werden. Die Schwangerschaftsrate pro Zyklus lag in den ersten 4 Zyklen bei 9,4 ± 11,7 % und nahm dann ab. Das Alter der Frau beeinflusste die Schwangerschaftsrate pro Zyklus (13,0, 10,6 bzw. 6,8% bei einem Alter von £ 30, 31 ± 35, 36 ± 40 Jahren). Die Kinderwunschdauer beeinflusste die Schwangerschaftsrate, jedoch ohne statistische Signifikanz zu erreichen. Die zugrunde liegende Sterilitätsursache beeinflusste die kumulative Schwangerschaftsrate (43,3 % bei endokriner Sterilität, 26,2 % bei idiopathischer Sterilität, 14,9 % bei andrologischer Sterilität). Die Schwangerschaftsrate war stark abhängig von den Spermiogramm-Parametern. Die Schwangerschaftschance nahm ab, wenn die Spermienkonzentration im Nativ-Spermiogramm < 20 Millionen/ml und in der aufbereiteten Probe < 10 Millionen/ml lag, der Anteil an Spermien mit normaler Morphologie < 5% war, der Anteil motiler Spermien in der aufbereiteten Probe < 50 % betrug oder die Anzahl motiler Spermien in der aufbereiteten Probe < 10 Millionen/ml betrug.

Success Rate of Inseminations Dependent on Maternal Age? An Analysis of 4246 Insemination Cycles
