Prospective evaluation of computed tomography and mediastinoscopy in mediastinal lymph node staging

Abstract: Twenty five (25%) mediastinoscopies were positive and three were false-negative (3%). There were 29 false-positive CT scans and 12 false-negative. Overall sensitivity and specificity of CT were 63 and 57%, respectively, and of mediastinoscopy 89 and 100%, respectively. Positive and negative predictive values of CT were 41 and 77%, respectively, and of mediastinoscopy 100 and 96%, respectively. Accuracy of CT was 59% and of mediastinoscopy 97%. Accuracy of CT was lowest for left-sided and centrally located tumo… Show more

“…Accuracy of CT in preoperative mediastinal lymph node staging was 45 to 79% (11)(12)(13)(14)(15). Recently, cervical mediastinoscopy or positron emission tomography (PET) scanning with 2-(fluorine-18) fluoro-2-deoxy-D-glucose (FDG) proved more accurate than computed tomography (CT) in demonstration and staging of nodal involvement (11)(12)(13).…”

“…In the present study, metastatic lymph nodes were defined as mediastinal lymph nodes larger than 10 mm along the short axis on CT scan. However, in the diagnosis of mediastinal lymph node metastasis with CT scan, with a sensitivity of 40-75% and a specificity of 40-66%, is inferior to mediastinoscopy, with a sensitivity of 80% to 90% and a specificity of 100%, or to positron-emission tomography, with a sensitivity of 90% and a specificity of 86% (24)(25)(26). Additionally, the exact stage of T3 or T4 tumors is difficult to determine with a CT scan.…”

“…O acesso aos linfonodos subcarinais e aos da janela aorto-pulmonar somente é conseguido através da mediastinostomia anterior. A mediastinoscopia tem rendimento diagnóstico que varia de 89 a 100% (48) . Ambos os procedimentos são realizados sob anestesia geral com mí-nima morbidade e mortalidade: respectivamente, 0,6% e 0,2% (49) .…”
