Pulmonary Metastasis of Polymorphous Low-Grade Adenocarcinoma of the Minor Salivery Gland

Abstract: A surgical case of pulmonary metastases of polymorphous low-grade adenocarcinoma (PLGA) originating from the minor salivary gland in the soft palate in a 62-year-old woman is reported. PLGA has been to be a locally invasive carcinoma without distant metastases; thus our case is the first reported case with histologically-proven distant metastases to the lung. We emphasise that attention should be paid to distant metastases especially to the lung even in case of PLGA.

“…4 -8 Its behavior is indolent, with a 10 -15 % rate of local recurrence, a regional lymph node metastasis rate of approximately 10%, and rare reports of distant spread. 9,10 These tumors are characterized by architectural diversity and cytologic uniformity. The nuclei are oval to spindled with fine chromatin and inconspicuous nucleoli.…”

“…12 In most cases, PLGA has a good prognosis after local treatment, however, the tumors have a 10-15% rate of local recurrence, a regional lymph node metastasis rate of approximately 10% and, rarely, distant metastasis. 5,9,[13][14][15] Histologically, these tumors are characterized by nonencapsulated, infiltrative borders; cytologic uniformity; and a variety of morphologic patterns, including tubular, solid, papillary, microcystic, Adenocarcinoma of Salivary Gland pseudoadenoid cystic, cribiform, fascicular, single file and strandlike. Experience with the cytology of this tumor is limited.…”

“…There is neither perineural, vascular nor lymph node metastasis in our patient as reported by Tanaka et al [10] and Kurmar et al [11]. Because of its morphologic pleomorphism, PLGA has often been wrongly diagnosed as pleomorphic adenoma or adenoid cystic carcinoma.…”
