Objective: To evaluate the anxiety experienced before, during and after conventional paraffin gauze nasal pack removal in patients operated under local versus general anesthesia. Study design: Comparative study. Place and duration of study: Department of ENT, Combined Military Hospital Sialkot and PAF Hospital Shorkot from July 2017 to June 2018. Material and methods: A total of 120 patients planned for Septoplasty were enrolled and divided into two groups. Sixty patients were to be operated under local anesthesia (Group A) while the other 60 were undergoing the same procedure under general anesthesia (Group B). Conventional paraffin gauze nasal packing was done for 24 hours in all 120 patients. Hamilton Anxiety Rating Scale (HAM-A) was used to determine the patients’ anxiety in both groups, 1 hour pre-operatively, immediately before and 1 hour after nasal pack removal. Results: The mean Hamilton Anxiety Scale assessment scores in both groups were of ‘mild’ category. The highest scores in both groups were observed immediately before nasal pack removal, with a range of 15-18, while the lowest scores in both groups were documented one hour after pack removal with a range of 13-16. Anxiety level in patients operated under general anesthesia was slightly lower than patients administered local anesthesia mean score of 16.40 ± 0.763 vs 17.21 ± 0.666 (p<0.001). Conclusion: Anxiety during nasal pack removal is mainly associated with prior pain experienced during nasal pack insertion. It is recommended that proper analgesia, adequate topical anesthesia, gentle insertion would make this process less distressing and will subsequently result in less anxiety at its removal.

Objective: Several types of nasal packs are used postoperatively in septoplasty. In this study, we compared two commonly used nasal packing materials, the intranasal septal splint with airway and Merocel tampon, in terms of pain, bleeding, nasal obstruction, eating difficulties, discomfort in sleep, and pain and bleeding during removal of packing in the early period. Methods:The study group included 60 patients undergoing septoplasty. Patients were divided into two groups (n=30 in each group). An intranasal splint with airway was used for the patients in the first group after septoplasty, while Merocel nasal packing was used for the second group. Patients were investigated in terms of seven different factors -pain, bleeding while the tampon was in place, nasal obstruction, eating difficulties, night sleep, pain during removal of the nasal packing, and bleeding after removal of packing. Results:There was no statistically significant difference between the groups in terms of pain 24 hours after operation (p=0.05), while visual analog scale (VAS) scores for nasal obstruction, night sleep, eating difficulties, and pain during packing removal were lower in the nasal splint group with a statistically significant difference (p<0.05). There was no statistically significant difference between the groups in terms of postoperative bleeding (p=0.23). Significantly less bleeding occurred during removal of the packing in the nasal splint group (p<0.05). Conclusion:Our study indicates that the nasal splint was more comfortable and effective in terms of causing lesser bleeding and pain during removal of packing.

Objective: To evaluate the effect of removal of nasal packs after sub mucous resection (SMR) on the first postoperative day versus second post operative day in terms of bleeding control and prevention of septal hematoma formation. Study Design: Comparative cross sectional study. Place and Duration of Study: ENT department, Combined Military Hospital Jhelum Pakistan, from Oct 2017 toOct 2018. Methodology: Total 100 patients of both genders, ages from 16 to 35 years with a deviated nasal septum wereincluded. Written informed consent was taken and hospital ethical committee approval was obtained. Patientswere randomly divided into two groups of 50 patients each by simple random sampling. Patients in both groupsunderwent submucous resection. Patients in whom sub mucous resection was combined with other procedureslike rhinoplasty, turbinectomy, polypectomy were excluded. In group A the nasal packs were removed on thefirst post operative day. In group B the nasal packs were removed on the second post operative day. Nasal packing in all patients was done with vaseline gauze rolls. The groups were compared for bleeding at the time of nasal pack removal and septal hematoma formation in the follow up period. Results: There was no statistically significant difference between the two groups in terms of bleeding (p-value0.40) and septal hematoma formation (p-value 0.49). Conclusion: After sub mucous resection, removal of nasal packs on 1st versus 2nd post operative day is notassociated with any significant difference in bleeding and septal hematoma formation.
