A capillary electrophoresis-time of flight-mass spectrometry (CE-TOF-MS) method for the analysis of amino acids in human urine was developed. Capillaries noncovalently coated with a bilayer of Polybrene (PB) and poly(vinyl sulfonate) (PVS) provided a considerable EOF at low pH, thus facilitating the fast separation of amino acids using a BGE of 1 M formic acid (pH 1.8). The PB-PVS coating proved to be very consistent yielding stable CE-MS patterns of amino acids in urine with favorable migration time repeatability (RSDs <2%). The relatively low sample loading capacity of CE was circumvented by an in-capillary preconcentration step based on pH-mediated stacking allowing 100-nL sample injection (i.e. ca. 4% of capillary volume). As a result, LODs for amino acids were down to 20 nM while achieving satisfactory separation efficiencies. Preliminary validation of the method with urine samples showed good linear responses for the amino acids (R(2) >0.99), and RSDs for peak areas were <10%. Special attention was paid to the influence of matrix effects on the quantification of amino acids. The magnitude of ion suppression by the matrix was similar for different urine samples. The CE-TOF-MS method was used for the analysis of urine samples of patients with urinary tract infection (UTI). Concentrations of a subset of amino acids were determined and compared with concentrations in urine of healthy controls. Furthermore, partial least squares-discriminant analysis (PLS-DA) of the CE-TOF-MS dataset in the 50-450 m/z region showed a distinctive grouping of the UTI samples and the control samples. Examination of score and loadings plot revealed a number of compounds, including phenylalanine, to be responsible for grouping of the samples. Thus, the CE-TOF-MS method shows good potential for the screening of body fluids based on the analysis of endogenous low-molecular weight metabolites such as amino acids and related compounds.

AbstrakInfeksi saluran kemih (ISK) merupakan salah satu penyakit infeksi yang memiliki prevalensi cukup tinggi. Berdasarkan data rekam medis RSUP dr. M. Djamil Padang pada tahun 2013, terdapat pasien ISK sebanyak 273 orang. Tujuan penelitan ini adalah untuk menganalisis rasionalitas terapi antimikroba parenteral serta pengelolaannya di bangsal penyakit dalam RSUP dr. M. Djamil Padang. Penelitian ini merupakan penelitian observasional menggunakan data prospektif pada bulan April sampai Juni 2015 di bangsal penyakit dalam. Kriteria inklusi yaitu pasien ISK dewasa (>18 tahun), dirawat di bangsal penyakit dalam RSUP dr. M. Djamil, serta mendapatkan terapi antimikroba parenteral. Hasil analisis penggunaan antimikroba parenteral menunjukkan bahwa antibiotik sefalosporin generasi III paling banyak digunakan (79,16%) serta dikombinasi dengan quinolon, flukonazol, dan metronidazol. Hasil analisis rasionalitas memperlihatkan bahwa terapi telah tepat indikasi, tepat pasien, tepat obat, dan tepat obat dan penggunaannya (94,7%). Pada proses pengelolaan sediaan antimikroba parenteral ditemukan bahwa penyimpanannya telah tepat, namun proses rekonstitusi belum memenuhi teknik aseptis serta prosedur yang telah ditetapkan oleh Departemen Kesehatan tahun 2009. Based on the Medical Record Data, there were 273 patients with UTI. The purpose of the study was to analyse the rationality of parental antimicrobial preparation use and management. This prospective observasional study used data from April to June 2015 in internal medicine wards. The inclusion criteria include UTI adult patients (>18 years), were treated at the department of internal medicine ward dr. M. Djamil hospital, as well as parenteral antimicrobial therapy. Results of the statistical analysis showed that the use of antimicrobial parenteral used of third generation cephalosporin antibiotik most widely used was (79.16%) and in combination with quinolones, fluconazole, and metronidazole. Results of the analysis showed that the therapy, patient, frequency, dosage and precise method of drug administration was (100%) duration of therapy was (94,7%) appropriate. As for the process management of parenteral antimicrobial preparations was found that the storage was right, but the reconstitution process has not complied with the procedures established by the Ministry of Health in 2009. The use of parenteral antimicrobial in patients with UTIs at dr. M. Djamil Padang hospital has been rational, but the reconstitution process is not appropriate the procedures established by the Ministry of Health in 2009.

ÖzetAmaç: Üriner sistem infeksiyonları (ÜSİ), yetişkinlerde bakteriyel infeksiyonların en sık görülen sebebi olup, her iki cinste ve tüm yaş gruplarında görülebilmektedir. Bu çalışmada, ÜSİ'nin hızlı ve güvenilir tanısında, Gram boyaması, Thoma sayma kamarasında lökosit sayımı, nitrit testi ve lökosit esteraz (LE) testinin etkinliği, ÜSİ tanısında altın standard olan idrar kültürüyle karşılaştırılmıştır. Yöntemler: Mayıs-Kasım 2010 arasında polikliniklerden ya da yatan hastalardan gönderilen 658 idrar örneği incelenmiştir. Klinik bilgilerine ulaşılan, ÜSİ'yi düşündüren semptomları bulunan ve yaş aralığı 0-106 olan hastalar çalışmaya dahil edilmiştir. İdrar örnekleri, Gram boyaması, Thoma sayma kamarasında lökosit sayımı, nitrit testi ve LE testiyle incelenerek kültürleri yapılmıştır. Üreyen mikroorganizmalar klasik yöntemler ve API ® (bioMeriéux, Fransa) kullanılarak saptanmıştır. Bulgular: Örneklerin 137 (%20.8)'sinde üreme tespit edilmiş, 143 (%21.7)'ü kontaminasyon olarak değerlendirilmiştir. Gram boyamasının duyarlılığı %82.2, özgüllüğü %96.8, yalancı pozitifliği %3.2, yalancı negatifliği %17.8 olarak saptanmış olup, bu oranlar sırasıyla Thoma sayma kamarasında lökosit sayımı için, %78.8, %81.5, %18.5, %21.2; nitrit testi için, %40.1, %95.8, %4.2, %59.9; LE testi için, %88.1, %34.1, %65.9, %11.9 olarak tespit edilmiştir. Sonuçlar: Gram boyamasının, duyarlılık ve özgüllüğünün oldukça yüksek, yalancı pozitif ve yalancı negatifliklerinin diğer yön-temlere oranla düşük ve uygulanabilirliğinin kolay olması nedeniyle, ÜSİ'nin hızlı tanısında kullanılabilecek en uygun yöntem olduğu sonucuna varılmıştır. AbstractObjective: Urinary tract infections are the most frequent bacterial infections in adults and may be seen in all age groups and both genders. In this study, urine culture which is the gold standard diagnostic method of urinary tract infections is compared with Gram stain, Thoma counting chamber, nitrite reductase test and leukocyte esterase (LE) test in efficiency for rapid and reliable diagnosis of urinary tract infection. Methods: This study was performed in our laboratory between May 2010 and November 2010. 658 urine samples of patients with urinary tract infection symptoms from outpatient and inpatient clinics were included in the study. Urine samples were examined by Gram stain, leukocyte count in Thoma chamber, nitrit test and LE test, and cultured. Microorganisms were identified using conventional methods and API ® (bioMeriéux, Fransa) identification systems. Results: Cultures were found positive in 137 (20.8%) samples and 143 (21.7%) cultures were evaluated as contamination. For Gram stain, the results for sensitivity, specificity, false positivity and false negativity were 82.2%, 96.8%, 3.2%, and 17.8%, respectively.
