Articles were retrieved from Medline, Scopus, Web of Science, Academic Search Complete (via Ebsco), and ProQuest with a number of key words and various combinations. Articles from Indonesian journals were also searched for with Google Scholar. A total of 56 articles from peer-reviewed journal databases and 19 articles from Indonesian journals were reviewed. Quantitative designs were applied more frequently than qualitative, and mixed methods designs were used in some studies. The majority gathered retrospective information about people's behaviors. Communicable diseases and maternity care were the most frequently studied conditions, in contrast to noncommunicable diseases. In terms of geographical distribution, most research was conducted on Java island, with very few in outside Java. Important findings are a model of Indonesian careseeking pathways, an understanding of determinants of people's care choices, and the role of sociocultural beliefs. The findings from this narrative review provide insight to what and how Indonesians make decisions to manage their illness and why. This makes an important contribution to understanding the problem of underutilization of medical services despite the government's extensive efforts to improve accessibility.

Angka penemuan Tuberkulosis (TB) tahun 2016 adalah sebesar 77% di dunia, sebesar 46,5% di Asia Tenggara dan sekitar 32 - 33% di Indonesia. Di Kota Depok angka penemuan TB mencapai 58%. Sektor swasta menjangkau 18,7% kasus TB di Kota Depok meskipun baru 40% RS swasta yang terlibat. Penelitian ini bertujuan untuk mengetahui apakah penerapan strategi DOTS di Rumah Sakit swasta Kota Depok lebih menghemat biaya dibandingkan di Puskesmas. Penelitian dilakukan selama 6 bulan dengan kohort retrospektif di Puskesmas DOTS, RS DOTS dan RS Non DOTS menggunakan 36 sampel per kelompok. Penghitungan dari perspektif societal dengan microcosting berdasarkan tarif, harga pasar, serta nilai anggaran. Outputnya angka pengobatan lengkap (Success Rate). Hasil penelitian menunjukkan Success Rate di puskesmas paling tinggi yaitu 86,1%, RS DOTS sebesar 77.78 % dan Non DOTS 63.89 %. Penambahan biaya provider terutama tenaga pelaksana khusus di puskesmas dan RS DOTS meningkatkan success rate. Biaya societal di puskesmas 42% dari biaya di RS swasta. ACER (Average Cost Effectiveness Ratio) menunjukkan RS yang melaksanakan strategi DOTS lebih cost effective. Untuk menaikkan 1% angka kesuksesan pengobatan membutuhkan biaya Rp 10.084.572 dengan melakukan intervensi program DOTS ke RS Swasta. Uji t independen menyatakan bahwa terdapat perbedaan bermakna biaya societal pengobatan tuberkulosis antara puskesmas, RS DOTS, dan RS Non DOTS.

ObjectiveThe research aimed to investigate the effect of lay knowledge of pulmonary tuberculosis (TB) and prior contact with pulmonary TB patients on a health-belief model (HBM) as well as to identify the social determinants that affect lay knowledge.MethodsSurvey research design was conducted, where participants were required to fill in a questionnaire, which measured HBM and lay knowledge of pulmonary TB. Research participants were 500 residents of Semampir, Asemrowo, Bubutan, Pabean Cantian, and Simokerto districts, where the risk of pulmonary TB transmission is higher than other districts in Surabaya.ResultsBeing a female, older in age, and having prior contact with pulmonary TB patients significantly increase the likelihood of having a higher level of lay knowledge. Lay knowledge is a substantial determinant to estimate belief in the effectiveness of health behavior and personal health threat. Prior contact with pulmonary TB patients is able to explain the belief in the effectiveness of a health behavior, yet fails to estimate participants’ belief in the personal health threat.ConclusionHealth authorities should prioritize males and young people as their main target groups in a pulmonary TB awareness campaign. The campaign should be able to reconstruct people’s misconception about pulmonary TB, thereby bringing around the health-risk perception so that it is not solely focused on improving lay knowledge.
