Treatment of Urinary Tract Infections with Canephron® in Germany: A Retrospective Database Analysis

Objective: The goal of the present study was to evaluate treatment with Canephron® compared to standard antibiotic treatment after diagnosis of acute cystitis or urinary tract infection (UTI), with regard to the risk of sporadic recurrent UTIs, frequent recurrent UTIs, UTI-related sick leave, additional antibiotic prescriptions, and renal complications (pyelonephritis). Methods: This retrospective cohort study was based on data from the IMS® Disease Analyzer database (IQVIA), and included outpatients in Germany with at least one diagnosis of acute cystitis or UTI with a prescription of either Canephron® or standard antibiotics between January 2016 and June 2019 and treated in general practitioner (GP), gynecologist, or urologist practices, from which the data were obtained. Multivariable regression models were used to investigate the association between Canephron® prescription and the amount of sporadic or frequent recurrent UTIs, as well as the duration of UTI-related sick leave, the number of additional antibiotic prescriptions, and cases of pyelonephritis. The effects of Canephron® were adjusted for age, sex, insurance status, and Charlson comorbidity score (CCI). Results: 2320 Canephron® patients and 158,592 antibiotic patients were available for analysis. Compared to antibiotic prescription, Canephron® prescription was significantly associated with fewer sporadic recurrences of UTI infections 30–365 days after the index date (odds ratio (OR): 0.66; 95%, confidence interval (CI): 0.58–0.72), as well as less frequent recurrences of UTI infections (OR: 0.61; 95% CI: 0.49–0.88), and also with reduced additional antibiotic prescription within 31–365 days (OR: 0.57; 95% CI: 0.52–0.63). No significant differences were observed between the Canephron® and antibiotic cohorts with regard to the likelihood of sick leave (OR: 0.99; 95% CI: 0.86–1.14), new antibiotic prescription within 1–30 days (OR: 1.01; 95% CI: 0.87–1.16), or occurrence of pyelonephritis (Hazard Ratio (HR): 1.00; 95% CI: 0.67–1.48). Conclusion: These real-world data show that Canephron® is an effective, safe symptomatic treatment for acute cystitis or UTI. It should be considered as an alternative treatment, particularly to also strengthen antimicrobial stewardship strategies.

The fruits of marsh cranberries due to the content of phenolic compounds, in particular proanthocyanidins, are effective in the prevention and treatment of cystitis [1]. The aim of the research was to study the anti-inflammatory properties of condensed juice of Vaccinium oxycoccos fruits and granules “Cranberry” in a model of acute bacterial cystitis in rats. Materials and methods. The anti-inflammatory properties of condensed juice from Vaccinium oxycoccos fruits and granules “Cranberry” were studied in a model of acute bacterial cystitis in rats, which was caused by three daily transurethral injection into the bladder of 0.3 ml of E. coli strain at a concentration of 106 CFU/ml. “Canefron® H” was used as a reference drug. Tissues were studied histologically by assessing the structure of the bladder wall and the condition of blood vessels. Results. Infection of animals with E. coli resulted in a significant increase in the desquamation of the transitional epithelium, edematous changes and narrowing of blood vessels in the tissues of the bladder wall. During treatment with the drug “Canefron® N” there was a decrease in cell infiltration and desquamation of cells, dilation of blood vessels. When animals used condensed juice from Vaccinium oxycoccos fruits and granules “Cranberry” in the histological picture of the walls of the bladder there was a decrease in desquamation, edema and cellular infiltration, the lumen of blood vessels was approaching normal. Conclusions. The use of condensed juice from Vaccinium oxycoccos fruits and granules “Cranberry” was accompanied by a positive effect on the tissues of the walls and blood vessels of the bladder of rats (there was a decrease in desquamation of the superficial layers of the transitional epithelium and normalization of the histological picture of vascular entanglement, that was not inferior to the comparison drug “Canefron® N”.

Non-antibiotic prevention of postcoital cystitis in women

Background: postcoital cystitis ranks first among recurrent infections of the lower urinary tract in young women. Their prevention is an important medical social issue. Prescription of antimicrobials for these infections increases antibacterial resistance of urinary tract pathogens, thereby requiring the search for alternative preventive strategies for postcoital cystitis. Aim: to assess the effectiveness of diet correction using biologically active food supplement (BAFS) Ecocystin® containing D-mannose and inulin to prevent coitus-related infections of the lower urinary tract (postcoital cystitis) in women. Patients and Methods: 40 sexually active women aged 18–45 (the mean age 31.9±8.8 years) with recurrent coitus-related infections of the lower urinary tract were enrolled. Women were divided into two groups, i.e., the study group (n=22) and the control group (n=18). Study group women received postcoital preventive treatment for the lower urinary tract infections (one sachet of BAFS Ecocystin® containing 1.5 g of mannose and 1.5 g of inulin no later than 2 hours after sexual intercourse and then another 12 hours later). Control group women received no preventive treatment. The primary indicator of the efficacy of postcoital prophylaxis was the rate of recurrent infections of the lower urinary tract. The additional indicator was the severity of clinical presentations assessed by the Acute Cystitis Symptom Score (ACSS) questionnaire. Follow-up was six months. Results: the rate of recurrent infections of the lower urinary tract was significantly lower in the study group compared to the control group. Recurrences were reported in 27.3% in the study group and 77.8% in the control group over months 1–3 (χ2=10.01, p=0.002) and 13.6% and 72.2%, respectively, over months 4–6 (χ2=14.16, p<0.001). The chances for cystitis exacerbation during the first and the second trimester of the follow-up were 2.9- and 5.3-times lower in the study group compared to the control group. During the first trimester of the follow-up, the recurrence rate was 0.45±0.86 per one woman of the study group and 1.55±1.10 per one of the control group (t=3.6, p<0.001). During the second trimester of the follow-up, the recurrence rate was 0.18±0.50 and 1.50±1.20 (t=4.7, p<0.001), respectively. Women who received BAFS Ecocystin® but developed postcoital cystitis reported much lower symptom severity than the control group. As a result, the study group’s quality of life was significantly higher than the control group’s. Conclusions: BAFS Ecocystin® is a highly effective and well-tolerated preventive tool for postcoital cystitis in women and a pathogenetically reasonable alternative for antibiotics.
