AbstrakTujuan Untuk menganalisis apakah bising pesawat terbang denyut nadi istirahat, dan faktor lain berperan meningkatkan risiko kejadian tekanan darah diastolik (TDD) pada penerbang TNI AU. Metode Desain penelitian nested case-control. Data berasal dari rekam medis hasil pemeriksaan kesehatan berkala dan indoktrinasi latihan aerofi siologi di LAKESPRA Saryanto di Jakarta antara Januari 2003 -September 2008. Kelompok kasus ialah subjek dengan TDD ≥90 mmHg; kelompok kontrol adalah dengan TDD ≤89 mmHg. Setiap kasus dipadankan dengan 12 kontrol. Hasil Di antara 567 penerbang, 544 (95,9%) mempunyai rekam medis yang lengkap, dan diperoleh 40 kasus dan 480 kontrol. Penerbang yang biasa terpajan bising dalam pesawat 90-95 dB dibandingkan 70-80 dB mempunyai risiko 2,7 kali menderita TDD tinggi [rasio odds (ORa) = 2,70; 95% interval kepercayaan (CI) = 1, [5][6]97]. Penerbang dengan frekuensi nadi istirahat ≥ 81/menit dibandingkan ≤ 80/minute mempunyai risiko 2,7 kali TDD tinggi (ORa = 2,66; 95% CI = 1,61). Ditinjau dari segi total jam terbang, penerbang dengan total jam terbang 1401-11125 jam dibandingkan 147-1400 jam mempuyai risiko 3,2 kali menderita TDD tinggi (ORa = 3,18; 95% CI = 1,01-10,03).Kesimpulan Intensitas bising dalam pesawat yang tinggi, frekuensi nadi istirahat yang tinggi, jumlah jam terbang total yang tinggi meningkatkan risiko TDD tinggi. Pemeriksaan mawas diri frekuensi nadi istirahat oleh penerbang dapat dipakai untuk pengendalian TDD tinggi. (Med J Indones 2009; 18: 276-82) AbstractAim To analyze the effects of aircraft noise, resting pulse rate, and other factors on the risk of high diastolic blood pressure (DBP) in Indonesian Air Force pilots.Methods A nested case-control study was conducted using data extracted from annual medical check-ups indoctrination aerophysiologic training records at the Saryanto Aviation and Aerospace Health Institute (LAKESPRA) in Jakarta from January 2003 -September 2008. For analysis of DBP: the case group with DBP ≥ 90 mmHg were compared with contral group with DBP < 79 mmHG. One case matched to 12 controls. ResultsOut of 567 pilots, 544 (95.9%) had complete medical records. For this analysis there were 40 cases of high DBP and 480 controls for DBP. Pilots exposed to aircraft noise 90-95 dB rather than 70-80 dB had a 2.7-fold increase for high DBP [adjusted odds ratio (ORa) = 2.70; 95% confi dence interval (CI ) = 1.05-6.97]. Pilots with resting pulse rates of ≥ 81/minute rather than ≤ 80/minute had a 2.7-fold increase for high DBP (ORa = 2.66; 95% CI = 1.26-5.61). In terms of total fl ight hours, pilots who had 1401-11125 hours rather than 147-1400 hours had a 3.2-fold increase for high DBP (ORa = 3.18; 95% CI = 1.01-10.03). ConclusionHigh interior aircraft noise, high total fl ight hours, and high resting pulse rate, increased risk for high DBP. Self assessment of resting pulse rate can be used to control the risk of high DBP. (Med J Indones 2009; 276: 276-82)
In cardiovascular surgery ischemia-reperfusion injury is a challenging problem, which needs medical intervention. We investigated the effects of curcumin on cardiac, myocardial, and mitochondrial parameters in perfused isolated working Guinea pig hearts. After preliminary experiments to establish the model, normoxia was set at 30 minutes, hypoxia was set at 60, and subsequent reoxygenation was set at 30 minutes. Curcumin was applied in the perfusion buffer at 0.25 and 0.5 μM concentrations. Cardiac parameters measured were afterload, coronary and aortic flows, and systolic and diastolic pressure. In the myocardium histopathology and AST in the perfusate indicated cell damage after hypoxia and malondialdehyde (MDA) levels increased to 232.5% of controls during reoxygenation. Curcumin protected partially against reoxygenation injury without statistically significant differences between the two dosages. Mitochondrial MDA was also increased in reoxygenation (165% of controls), whereas glutathione was diminished (35.2%) as well as glutathione reductase (29.3%), which was significantly increased again to 62.0% by 0.05 μM curcumin. Glutathione peroxidase (GPx) was strongly increased in hypoxia and even more in reoxygenation (255% of controls). Curcumin partly counteracted this increase and attenuated GPx activity independently in hypoxia and in reoxygenation, 0.25 μM concentration to 150% and 0.5 μM concentration to 200% of normoxic activity.
Diet mengendalikan hipertensi orang Indonesia di Jakarta (dietary approach to stop hypertension for Indonesian at Jakarta, disingkat DASHI-J) dan olahraga jalan cepat dapat mengurangi berat badan dan menurunkan tekanan darah pada pasien prahipertensi. Tujuan penelitian ini adalah mengevaluasi diet dan olahraga tersebut dengan desain penelitian clinical trial terhadap 100 laki-laki berusia 25 – 55 tahun di PT Krama Yudha Ratu Motor. Responden dialokasikan secara acak dalam 4 kelompok perlakuan meliputi kelompok diet (A), kelompok olahraga jalan cepat (B), kelompok diet dan olahraga jalan cepat (C), dan kelompok kontrol (D). Kelompok A dan C menerima diet 5 hari dalam seminggu selama 8 minggu. Kelompok diet DASHI-J diberikan makan siang dan makan malam. Setelah 2 bulan intervensi, berat badan, indeks massa tubuh, visceral fat, body fat, lingkar perut, tekanan darah sistolik dan diastolik, kolesterol serum dari semua kelompok menurun secara signifikan. Penurunan tertinggi terjadi pada kelompok diet DASHI-J dan olahraga jalan cepat dengan penurunan berat badan 4,18 kg, indeks massa tubuh 1,50 kg/m2, tekanan darah 12,00 mmHg/8,60 mmHg. Diet DASHI-J dan olahraga jalan cepat berperan menurunkan berat badan, indeks massa tubuh, serta tekanan darah sistolik dan diastolik.Dietary approach to stop hypertension for Indonesian at Jakarta (DASHI-J) and brisk walking exercise could reduce body weight and blood pressure of males prehypertension. The objective of the study was to evaluate DASHI-J by an experimental clinical trial conducted with 100 male subjects, aged 25 – 55 years divided randomly into 4 groups: DASHI-J diet group (A), brisk walking exercise group (B), DASHI-J and brisk walking exercise group (C), and control group (D). Group A and C got 5 days a week for 8 weeks diet. DASHI-J group was given lunch and dinner. After 2 months of intervension, the body weight, body mass index, body fat, visceral fat, waist circumference, systole, diastole, cholesterol serum of the groups all reduce significantly. The highest reduction of those intervention achieved by DASHI-J and brisk walking exercise group with body weight decreased 4.18 kg, Body Mass Index (BMI) 1.50 kg/m2, blood pressure 12.00/8.60 mmHg. This implies that DASHI-J and brisk walking exercise play a significant role in reducing body weight, BMI, and both sistolic and diastolic blood pressure.
Oxygen, although is highly required for living, can be toxic because it can be easily changed into oxygen free radicals (reactive oxygen species, ROS) and generates oxidative stress. [1][2][3][4][5] This oxidative stress will cause endothelial lesion and will be responded by endothelial cells through nitric oxide (NO) response and immunological response pathway. 6-8 The immunological response begins by increasing cytokines production, activated coagulation factors, and reactivation of platelet in acute thrombotic process. There is no data about the association of ROS and NO after short-term low-intensity exercise training within 5 days in patients hospitalized with acute myocardial infarction (AMI). A total of 32 male patients with AMI were randomized into two groups: 15 patients with short-term low-intensity exercise training within 5 days formed the training group and 17 patients without such exercise training formed the control group. All patients performed exercise treadmill test with modified Bruce protocol before and after the study. F2-isoprostane and NO concentration of the training group increased slightly after modified Bruce exercise treadmill test. Compared with the control group, NO of the training group was also slightly higher. Baseline NO and uric acid were negative predictor variables for F2-isoprostane in all patients hospitalized with AMI, and triglyceride was a positive predictor variable. After the study, physical capacity of the training group was higher; but heart rate and systolic blood pressure were lower significantly. This study showed that short-term low-intensity exercise training for patients hospitalized with AMI did not change ROS and NO productions, but it improved physical capacity and lowered heart rate and systolic blood pressure. NO was negative predictor variable for F2-isoprostane in controlling ROS changes in dynamic compensation mechanism.
