A report on 58 reconstructive vascular operations in chronic blocks of arteries of the leg. 4 different types of block in these arteries can be distinguished. Operation is indicated only in severe disturbances of circulation of stage III and IV. The criterion for reconstruction is not found in the proximal part of the arteries. Angiographic proof of at least one leg-artery down to the periphery being patent and permitting free flow is required. The various techniques applicable to each type of block, early and late results are reported on. Suring a period of up to 3 years block recurred in 26%. Most recurrences (46%) were seen within the first 30 days.
Zusammenfassung Es wird über die Verlaufsanomalie der A. carotis interna bei 13 operierten Patienten berichtet. Morphologisch können 3 verschiedene Formen unterschieden werden, und zwar eine vermehrte Schlängelung, eine extreme Schlingenbildung und die Knickstenose. Die Operationsindikation ist nur dann gegeben, wenn zerebrale Ausfallserscheinungen bestehen. Eine prophylaktische Korrektur ohne neurologische Symptome ist nicht indiziert. Über die eigene Operationstechnik (Resektion mit Patchplastik) wird berichtet. Die 11 nachuntersuchten Patienten waren bis auf geringe diskrete neurologische Restsymptome beschwerdefrei. Anomalies in the Course of the Internal Carotid Artery (Operative Indication, Technique and Results) Anomalies in the course of the internal carotid artery in 13 operated patients are reported. Three different forms can be distinguished morphologically: increased meandering, extreme loop formation and flexion stenosis (stenosis due to kinking). Operation is only indicated if there are signs of disturbance of cerebral function. Prophylactic correction without neurological symptoms is not indicated. The author's operative technique (resection with patch-plasty) is reported. The 11 patients who were followed up were found to be free of complaints apart from slight residual neurological symptoms.
