In this study we aimed to evaluate the influence of obesity in kidney and patient survival and graft function. Retrospective cohort study of kidney transplant recipients performed between 2001 and 2009. The body mass index was calculated at time of transplantation, one and five years after. The main outcomes studied were incidence of delayed graft function, new onset diabetes after transplantation, patient and graft survival, and glomerular filtration rate. The prevalence of obesity and overweight patients were 10.7% and 26.8% respectively, with an increase to 16.9% and 32.5% one year after transplantation. Underweight and obese recipients presented a higher incidence of early graft loss. The incidence of new onset diabetes after transplantation was significantly higher at one and five years in overweight or obese recipients at baseline. Overweight and obese recipients presented significantly lower estimated glomerular filtration rate at five years posttransplantation (p = 0.002). In the Kaplan-Meier analyses no statistically significant differences in patients or grafts survivals were observed. Obese patients have a higher rate of early graft failure and a higher new onset diabetes after transplantation incidence. Also, the finding of decreased glomerular filtration rate is worrisome and perhaps longer follow-up will reveal more graft failures and patients deaths in the group of obese recipients.
Resumo: Este artigo tem por objetivo apresentar uma metodologia para avaliação dos periódicos científicos na área da Educação no Brasil. Trata-se de um esforço inicial na direção de discutir aspectos importantes para tal avaliação e contribuir com o debate no processo de elaboração de critérios para o Qualis Periódicos da área na CAPES. A proposta aqui desenhada, de construção de um indicador, destaca quatro elementos centrais para a avaliação: Periodicidade/Atualização, Internacionalização, Origem e Distribuição Geográfica dos Autores e Fator de Impacto, com vistas a que possamos avaliar, quantificar, classificar e estratificar os periódicos considerando quão atuais, representativos, internacionais e lidos/citados são. ABSTRACTThe present work aims at presenting results of empirical research about the situations of labor and employment in institutions of child education in Brazil, highlighting the case of Belo Horizonte, Capital city ofMinas Gerais State, which offers nurseries and preschools and it is representative of Brazilian urban centers. An analysis including documental sources, legislation, statistics, interviews and observations was conducted, aiming at revealing the relations between the typologies of child education establishments and the forms of recruitment, career, salary and working conditions. The situations examined referred to public education institutions (State and Municipal) and private institutions, according to the categories private, communitarian, philanthropic and confessional. The studied establishments were selected, through an intentional sampling procedure, based on location and preferential public attended. Information was also collected from employers -public and private -and from representations of the unions/associations of professionals, in the public and private sectors. The existence of professionals with differentiated status and formation/ qualification was evident, as well as a number of modalities of labor and work relations, which reveal a process of precariousness in the professional exercise in child education. Cleavages within the public sector and between public and private sectors reiterate historical inequalities in that field. However, an increased demand for professionalization is observed.
