The G-1 column is an extracorporeal type granulocytapheresis device packed with 220 g cellulose acetate beads to which granulocytes and monocytes specifically adhere. A total of 59 rheumatoid arthritis patients with elevated granulocyte counts from 4 hospitals in Japan received 2 apheresis sessions of 1 h duratiodweek for a total of 8 times over a period of 4 weeks. About 55% of the leukocytes which entered the G-1 column were adsorbed onto the beads; 95% were granulocytes, 3.5% monocytes, and 0.4% lymphocytes. Clinical and efficacy assessments showed improvements in swollen joints (p < O.Ol), tender joints (p < 0.001), the active joint score (p < 0.001), duration of morning stiffness (p < 0.01), and grip strength (p < 0.001). In good responders, the improvements were observed for up to 12 weeks following the last apheresis. Exacerbation was noted in 2 patients. It is suggested that the efficacy of the G-1 column is attributable to the removal or suppression of hyperactive leukocytes and inflammatory cytokines, inducing a kind of immunomodulation.
Cytotoxic activity of polymorphonuclear leukocytes (PMN) in the peripheral blood of patients with various diseases was demonstrated to K562 cells (natural cytotoxicity, NC) and the antibody-coated P815 cells (antibody-dependent cellular cytotoxicity, ADCC, using a 51Cr-release method. The NC values of normal PMN were lower than those of normal lymphocytes with mean values of 5.0% and 30%, respectively. The NC values of patients' PMN were also lower in malignancy, chronic hepatitis and connective tissue diseases. The ADCC values of normal PMN were moderately high with a mean value of 16.0%, which was almost a half of normal lymphocytes. Higher ADCC values of PMN were found in patients with chronic hepatitis, SLE and Behcet's disease, and in these cases the ADCC values of their lymphocytes were extremely low. The supernatants of PMN mix-cultured with unlabeled K562 or the antibody-coated P815 cells were fairly cytotoxic to both cells, though the similar supernatants of lymphocytes were cytotoxic only to K562 cells, but not to the antibody-coated P815 cells. -NC; ADCC; lymphocytes; PMN; SLE Polymorphonuclear leukocytes (PMN) in the peripheral blood have been recently found to possess cytotoxic effect on the target cells either with direct contact (Hafemann and Lucas 1979) or in combination with antibody (Hafemann and Lucase 1979;Gale and Zighelboim 1975;Zighelboim et al. 1976;Clark and Klebanoff 1977). Pregnancy-associated α2-glycoprotein (PAG) was found to be increased in sera and synovial fluids of patients suffering from rheumatoid arthritis tested in the single radial immunodiffusion. The PAG levels in rheumatoid sera correlated well with the activity of arthritis in the patients. The purified PAG inhibited clearly the 3H-thymidine uptake of normal lymphocytes stimulated with phytohemagglutinin and also the spontaneous attachment of sheep erythrocytes to normal lymphocytes. 17 of 19 rheumatoid plasmas inhibited the 3H-thymidine uptake of normal lymphocytes and intensity of their inhibitory effects correlated with serum PAG levels.
