ABSTRAKPrevalensi penyakit diabetes melitus (DM) dan hipertensi di Indonesia semakin tahun semakin meningkat. Pemerintah melalui Badan Penyelenggara Jaminan Sosial (BPJS) mencanangkan Program Pengelolaan Penyakit Kronis (Prolanis) untuk mencapai kesehatan yang optimal dan kualitas hidup yang baik pada penyakit DM dan hipertensi. Studi ini bertujuan untuk mengukur kualitas hidup pasien prolanis di puskesmas menggunakan instrumen EQ-5D-5L. Studi dilakukan pada 200 pasien prolanis yang terdiri dari 100 pasien diabetes melitus dan 100 pasien hipertensi. Lokasi pengambilan sampel berada di puskesmas Kota Palu Provinsi Sulawesi Tengah untuk pasien DM dan Kabupaten Aceh Barat Provinsi Nangroe Aceh Darussalam untuk pasien hipertensi yang dilakukan dalam kurun waktu 3 bulan (September-November 2018). Penelitian menggunakan rancangan cross-sectional study dengan metode convenience/accidental sampling. Kualitas hidup diukur menggunakan instrumen European Quality of Life-5 Dimension-5 Level (EQ-5D-5L) dan konversi health utility (utility) dengan value set Indonesia. Analisis data menggunakan uji independent t-test untuk melihat perbedaan kualitas hidup pada tiap kelompok karakteristik pasien DM dan hipertensi. Hasil penelitian menunjukkan domain rasa sakit/tidak nyaman merupakan domain yang paling banyak dilaporkan terjadi masalah pada pasien DM maupun Hipertensi. Nilai utility pasien DM sebesar 0,843±0,081, sedangkan hipertensi sebesar 0,767±0,154. Terdapat perbedaan signifikan nilai utility berdasarkan karakteristik umur (p=0,001), riwayat penyakit keluarga (p=0,006), lama menderita penyakit (p=0,000) dan frekuensi kontrol (p=0,000) pada pasien hipertensi, sedangkan pada pasien DM hanya pada karakteristik memiliki penyakit lain (p=0,026). Quality of life (HRQoL) is a measure of a person's health in physical, spiritual, and emotional, and role functions in the society. Measurement of quality of life (HRQoL) is an important thing to understand and evaluate. Measurements are carried out not only on patients but also on the general population with the use of specific or generic instruments. The instrument used requires a psychometric properties test to ensure its validity and reliability. This article aims to conduct a systematic review of the psychometric properties of quality of life (HRQoL) instruments in the general population. Articles were collected in December 1st to 5th, 2018, from Pubmed and Google Scholar. The assessment was carried out using the checklist properties according to the cohen criteria and included the content validity, construct validity, internal consistency reliability, test-retest, ceiling effect and the level of credibility of the instruments. Among 80 articles obtained there were 24 articles that fulfilled the inclusion criteria. Short Form-36 (SF-36) instrument is the most widely used instrument in measuring the quality of life in the general population (26.6%). Some instruments have not been tested according to the criteria, while almost all instruments show a good level of validation of construct validity using convergent and discriminat validity with cronbach alpha values > 0.7. Test-retest reliability provides a good correlation. Some instruments show a ceiling effect. According to the assessment, the SF-36, SF-6D, EQ-5D, SF-12 and PedsQoL instruments are considered as established instruments. Further validation testing is needed to provide and support the measurement of subsequent quality of life (HRQoL) instruments.
Tuberkulosis (TB) menjadi salah satu masalah kesehatan masyarakat dunia. Saat ini angka kematian terhadap TB semakin meningkat di Indonesia. Dilaporkan kasus Tuberkulosis (TB) mencapai 420.994 kasus selama tahun 2017 dan sebanyak 8% dari total kasus seluruh dunia di tahun 2018. Perawatan terapi yang panjang pada pasien TB menyebabkan tingginya biaya pengobatan yang harus dibayarkan oleh pasien. Studi ini bertujuan untuk melihat besarnya biaya perawatan pada pasien tuberkulosis dari perspektif societal dan faktor yang mempengaruhinya. Studi dilakukan pada pasien TB di Rumah Sakit Paru Respira Yogyakarta dan Poli Paru di Puskesmas Sewon I yang dilakukan dalam kurun waktu 3 bulan (Januari-Maret 2019). Penelitian merupakan rancangan deskriptive dengan pendekatan cross-sectional. Tehnik sampling yang digunakan adalah accidental sampling pada 35 pasien. Analisis data secara deskriptif untuk memberikan estimasi biaya dan uji beda untuk melihat faktor yang mempengaruhi biaya pasien TB. Hasil penelitian menunjukan Biaya medis langsung merupakan komponen biaya yang paling tinggi dibandingkan komponen biaya lainnya. Biaya medis langsung pasien Tuberkulosis per episode rawat jalan sebesar Rp.106.745±77.050, non medis langsung Rp.26.024±31.247, biaya tidak langsung sebesar Rp.70.820±71.488. Faktor jenis pembiayaan (p=0.011) memberikan pengaruh signifikan terhadap biaya medis langsung. Faktor jenis kelamin (p=0.004) dan status bekerja (p=0.000) memberikan pengaruh signifikan terhadap biaya tidak langsung.
Breast cancer is the second most common cancer in the world and is one of the biggest causes of cancer deaths every year. Measurement of the quality of life in breast cancer patients could improve treatment and become a prognosic factor along with medical parameters for the patients. This study aims to measure the life quality of breast cancer patients and observe how it affects the life quality based on patient characteristics. The study was conducted on 93 breast cancer patients. Data were collected form patients in several breast cancer comunities in Denpasar which were carried outh within 3 months (January – March 2019). This study used a cross sectional design using a snowball sampling technique involving breast cancer patients in several cancer communities in Denpasar. The patient's life quality data was obtained using the EuroQol-5 Dimension-5 Level (EQ-5D-5L) generic instrument and conversion to health utility (utility) used the Indonesian value set. The entire data was analyzed using the independent t-test and multivariate regression. The results showed that pain/discomfort and anxiety/depression were the most dominant problem in breast cancer patients (82,7%). The utility value of breast cancer patients was 0.821±0.123 and the VAS value was 74.41±11.67. There was a significant difference in utility value based on patient characteristics of occupation (p=0.035), symptoms (p=0,003), and cancer stadium (p=0.015). Affected 20.5% of the quality of life of breast cancer patients in Denpasar city.
