2 ABSTRAK Latar Belakang: Keterlambatan penyerahan berkas klaim pasien rawat inap disebabkan karena berkas rekam medis yang kembali dari bangsal rawat inap tidak lengkap sehingga harus dikembalikan untuk dilengkapi oleh dokter penanggung jawab. Sedangkan keterlambatan klaim BPJS untuk pasien rawat jalan disebabkan karena persyaratan dan ketentuan administrasi klaim yang selalu berubah-ubah membuat banyak berkas klaim yang dikembalikan oleh verifikator sehingga prosesnya menjadi terhambat. Apabila terjadi keterlambatan penyerahan berkas klaim, maka berkas klaim tersebut akan di-pending penyerahanya dan akan diikutsertakan pada proses klaim bulan selanjutnya. Tujuan: mengetahui penyebab terjadinya keterlambatan penyerahan berkas klaim BPJS kepada verifikator independen di Rumah Sakit Panti Nugroho Metode: diskriptif dengan pendekatan kualitatif dan rancangan penelitian studi kasus. Subyek yang menjadi penelitian ini adalah petugas yang terlibat secara langsung dalam proses klaim. Obyek yang menjadi penelitian ini adalah pengajuan berkas klaim pasien rawat jalan dan rawat inap dengan cara bayar BPJS. Hasil: Proses pelaksanaan klaim BPJS di Rumah Sakit Panti Nugroho sudah lancar tetapi masih terdapat berkas klaim yang diserahkan terlambat. Faktor penyebab keterlambatan klaim berasal dari faktor man adalah petugas verifikator kelengkapan awal, dokter, dan petugas pengodean. Faktor machine adalah karena SIMRS belum terintegrasi dengan INA-CBG. Methode adalah karena implementasi SPO yang belum lancar. Faktor materials karena persyaratan yang tidak sesuai. Upaya yang telah dilakukan oleh petugas pada faktor man adalah melakukan sosialisasi pada pasien, berkomunikasi dengan dokter, mengupayakan perekrutan. Machine adalah melakukan back-up data. Faktor methode adalah membentuk tim BPJS di Rumah sakit Panti Nugroho. Kata Kunci: penyebab keterlambatan, klaim BPJS ABSTRACT Background: The late of inpatient BPJS claim caused by incomplete medical record so the medical record should returned back until responsible doctor completing that medical record. Complications are the factors that determine the length of the day of treatment of diabetes mellitus patients in the hospital such as hypertension, stroke, coronary heart disease, cholesterol, obesity and others. This study aims to analyze differences in Length of Stay (LOS) of patients with diabetes mellitus based on complications in RSUP Dr. Sardjito Yogyakarta.This research is an analytic observational quantitative research with cross sectional design. The population is all people with diabetes mellitus from year 2011-2016 recorded in electronic medical record. All the population used as research sample that is as many as 1,554 people. Independent variables in this study were hypertension, nephropathy, neuropathy, obesity, CHD, stroke, and pulmonary TB. Dependent variable is Length of Stay or long treatment of diabetes mellitus patient. Data analysis was performed using Independent T test. Diabetes mellitus patients at RSUP Dr. Sardjito year 2011-2016 amounted to 1,554 people with a trend that tends to decline. Most patients were aged 56-63 years (27.86%). The incidence of diabetes mellitus is dominated by type 2 with the highest complications being hypertension, nephropathy, and neuropathy. Based on the result of statistical test, there is a significant difference between the average LOS between patients with diabetes mellitus who have hypertension complications with non-hypertensive (p-value = 0.00), neuropathy with non-neuropathy (p-value = 0.00), coronary heart disease with which is not coronary heart disease (p-value = 0.035), and stroke with non-stroke (p-value = 0.032). Conclusions: There is significant difference in mean LOS among patients with diabetes mellitus who have complications of hypertension, neuropathy, coronary heart disease, and stroke with no complications of the disease.
The implementation of maintenance medical record documents in RSUP Dr. Sardjito is not optimal. This statement was proven by the damage of medical record documents about 35,05%. The purpose of this research is to review the implementation of maintenance medical record documents uses management elements 5M (man, money, material, machine, and method). The research type is qualitative and the data collection techniques use interviews, observations, and documentations. The subjects of this research are filling officers. The result from man factor shown that filling officers have not received maintenance medical record documents training, meanwhile money factor shown that filling officers did not know about the budget management of maintenance medical record documents. The result from material factor show that the materials for medical records forms and folders have been provided optimally, meanwhile machine factor shown that the type of rack from combination of iron and wood was less suitable because it can made medical record documents were damage. The result from method factor show that standard operational procedure about implementation of maintenance medical record documents was not available. The solutions are giving socialization about implementation of maintenance medical record documents to filling officers, providing standard operational procedure about implementation of maintenance medical record documents, coating around the sharp part of rack using rubber tires that can prevent the damage of medical record folder.
2 ABSTRAK Latar Belakang: Diabetes mellitus adalah penyakit kronis yang mempengaruhi beban ekonomi dan sosial secara luas. Data pasien dicatat melalui sistem rekam medis pasien yang tersimpan dalam database sistem informasi rumah sakit, data yang tercatat belum dianalisis secara efektif untuk menghasilkan informasi yang berharga. Teknik data mining bisa digunakan untuk menghasilkan informasi yang berharga tersebut. Tujuan: Mengidentifikasi karakteristik pasien Diabetes mellitus, kecenderungan dan tipe Diabetes melitus melalui penerapan teknik data mining di RSUP Dr. Sardjito Yogyakarta.Metode: Penelitian ini merupakan penelitian deskriptif observasional dengan rancangan cross sectional. Teknik pengumpulan data dilakukan secara retrospektif melalui observasi dan studi dokumentasi rekam medis elektronik di RSUP Dr. Sardjito Yogyakarta. Data yang terkumpul kemudian dilakukan analisis dengan menggunakan aplikasi Weka. Hasil: Pasien Diabetes mellitus di RSUP Dr. Sardjito tahun 2011-2016 berjumlah 1.554 orang dengan tren yang cenderung menurun. Pasien paling banyak berusia 56 -63 tahun (27,86%). Kejadian Diabetes mellitus didominasi oleh Diabetes mellitus tipe 2 dengan komplikasi tertinggi adalah hipertensi, nefropati, dan neuropati. Dengan menggunakan teknik data mining dengan algoritma decision tree J48 (akurasi 88.42%) untuk analisis rekam medis pasien telah menghasilkan beberapa rule. Kesimpulan: Teknik klasifikasi data mining (akurasi 88.42%) dan decision trees telah berhasil mengidentifikasi karakteristik pasien dan menemukan beberapa rules yang dapat digunakan pihak rumah sakit dalam pengambilan keputusan mengenai penyakit Diabetes mellitus. Occupational stress is recognized as a global health issue that affects all professions, no other profession in the health field i.e. medical record registration section. RSUP Dr. Sardjito Yogyakarta has a total data visit annually on an outpatient registration, which is in 2018 as much as 469813 and in 2019 as many as 518806. The number of visits explained at least the registration staff should serve 200-300 patients every day. Known errors that occur in SIMETRISS also harm the officer because inevitably officers should double job. The Stress of work felt by the officers of the list is often felt muscle tension, feel tense, fast angry, feel anxious, and difficult to concentrate in the work. Stressful work if not resolved will have an impact on activities in the scope or out of work. The research aims to figure out the work stress overview of the registration officers in RSUP Dr. Sardjito Yogyakarta. The collection methods used are interviews. The analysis of the data used in this study is qualitative. The results of the study showed work stress experienced by outpatient registration officers, that is caused by a physical work environment that is less comfortable to cause a lack of concentration by the officers and causes the performance of officers to decline, the conflict between family demands and the demands of agencies such as the use of less time breaks and the presence of a sustained shift, performance demands are done quickly and precisely however support facilities, namely Simetriss still error also raises pressure for the work RSUP Dr. Sardjito Yogyakarta has reduced some problems by conducting effective consultation on outpatient registration officers.
