The aim of this study was to determine the correlation between the level of glutathione reductase [GR] in relation with the severity of nuclear cataract and the increase of age. An analytic cross sectional observasional study of nuclear cataract patient seen at Saiful Anwar Hospital and Mass Cataract Surgery performed by Perdami Malang was carried out. Patients were classified into 3 groups based on age and 4 groups based on severity of nuclear cataract. The samples were taken by consecutive sampling and were operated on visual indication. The extracted lens were examined for [GR]. The laboratory findings of [GR] were compared with the severity of senile cataract. The data were analyzed with one way ANOVA. Confidence limit was set at 95%. A total of 55 patients were included with the sample size in 40-50 age group less than estimation. The [GR] were found to be significanfly lower in the older age (p=0,01) and the more severe cataract groups (p=0,00). The [GR] were also found to be significantly decreased with the increase of cataract severity in 40-50 age group (p=0,00), 50-60 age group (p=0,01) and >60 age group (p=0,00). In the grade 2 cataract, there were no significant difference of [GR] between age group (p=0.22). This also occured in grade 3 (p=0.23) while in the grade 4, there were significant difference between age group (p=0.01). This also occured in grade 5 (p=0.06). Pearson correlation test showed that there was a strong correlation between [GR] and increase of age (r=0.979) and cataract severity (r=0.969). But in the same cataract grade, the increase of age was not always associated with low [GR] r=0.413,for grade 2, r=0.876 for grade 4 and r=0.731 for grade 5). The conclusion is the increase of age and severity of nuclear cataract In senile cataract, are associated with low [GR]. This association was also found, with the increase of cataract severity in the same age group. ABSTRAKLensa tanam asferik dapat mengurangi aberasi sferik yang terjadi pasca operasi katarak, dan meningkatkan sensitivitas kontras dibandingkan lensa tanam sferik. Penelitian ini membandingkan sensitivitas kontras antara pasien yang diberi lensa tanam asferik dan sferik dengan menggunakan lembar kartu Pelli-Robson yang telah dilakukan dengan metode observasi cross sectional. Pemeriksaan dilakukan pada 32 mata yang diambil secara konsekutif dari 23 pasien minimal 1 bulan setelah dilakukan fakoemulsifikasi dan penanaman lensa tanam (asferik dan sferik). Sensitivitas kontras diperiksa pada jarak 1 m di depan lembar kartu dengan tajam penglihatan terbaik pasien setelah koreksi (log MAR) dan keratometri kurang dari 1,5Dioptri (D). Berdasarkan data demografi dan data operasi tidak didapatkan perbedaan yang signifikan, sensitivitas kontras lebih tinggi pada lensa tanam asferik dibandingkan sferik dengan independent t test. Perbedaan yang signifikan ini dapat menjadi pertimbangan untuk pemilihan lensa tanam yang sesuai dengan kebutuhan agar pasien mendapatkan fungsi penglihatan terbaiknya. ABSTRAKSalah satu faktor yang berperan pada katarak senilis adalah stres oksidatif. Penelitian ini bertujuan untuk mengevaluasi pengaruh ekstrak bilberry untuk mengurangi kadar malondialdehid lensa katarak senilis. Uji klinis tersamar ganda dilakukan dengan 33 orang pasien katarak senilis. Subjek dibagi menjadi 3 kelompok secara merata yaitu plasebo, perlakuan 100 mg ekstrak bilberry dan 300 mg ekstrak bilberry. Pengobatan yang diberikan sampai 14 hari sebelum operasi katarak untuk mengevaluasi tingkat malondialdehid. Data dianalisis menggunakan ANOVA dan uji Tukey. Tingkat rata-rata malondialdehid di kelompok plasebo, ekstrak bilberry 100 mg, dan 300mg adalah 11,82± SD 2,96 nmol/mg, 7,99±SD1,99 nmol/mg, dan 5,22±1,27SD nmol/mg. Efek penurunan malondialdehid dengan pemberian 100 mg dan 300 mg ekstrak bilberry berbeda signifikan (p<0,05). Terdapat korelasi negatif antara ekstrak bilberry dan malondialdehid tergantung dosis (r=-0,760; p<0,001). Diagram menunjukkan bahwa peningkatan dosis ekstrak bilberry akan mengakibatkan penurunan tingkat malondialdehid pada pasien katarak senilis. Dapat disimpulkan bahwa pemberian ekstrak suplemen bilberry dapat mengurangi kadar malondialdehid di lensa pasien katarak senilis dan peningkatan dosis ekstrak bilberry berkorelasi dengan tingkat penurunan malondialdehid pada lensa penderita katarak senilis. (r=-0,760; p<0,001). The higher the extract bilberry dose the lower the malondialdehyde level in senile cataract lens (r=-0,760, p=0,000). It can be concluded that extract bilberry supplementation can reduce the levels of malondialdehyde in lens of senile cataract patients.
Purpose: To report refractive management of bilateral lens subluxation case in Marfan Syndrome patient. Methods: Diagnosis based on a history‐taking, a physical examination, and an ancillary test. The refractive state of the patient was examined by an autorefractometer, streak retinoscopy and best‐corrected visual acuity (BCVA), before and after cycloplegic on each eye. Results: A 7‐years‐old girl diagnosed with Marfan Syndrome by the Department of Paediatric Cardiology complained for blurred vision in both eyes. Initial visual acuity of the patient was 0.027 in her right and left eyes. Anterior segment examination showed phacodenesis in both eyes. After cycloplegic refraction with enlarging pupil diameter and performed streak retinoscopy, BCVA in the right eye was 0.040 with a refractive correction of −22.00 diopters (D) and left eye was 0.021 with −20.00 D. After several follow ups, the refractive state remains highly fluctuates due to severe lens subluxation degree, which is seen through a half pupillary area. Consequently, operative approach using intracapsular lens extraction technique is considered. A month after the surgery, the distance visual acuity in right eye was 0.125 and 0.080 in left eye. With a refractive correction of +5.50 D, visual acuity became 0.500, while for the left eye became 0.125 with +5.00 D. Whereas, binocular distance visual acuity with refractive correction became 0.400. Therefore, spectacle was prescribed to achieve optimal visual acuity. Conclusions: The refractive management steps in lens subluxation associated with Marfan Syndrome case is very challenging depends on patient's condition. The intracapsular lens extraction procedure prior to spectacle prescription should be considered as a management of severe lens subluxation in patient with Marfan syndrome, in order to achieve BCVA.
Pendahuluan: Insiden katarak komplikata pada sindrom VKH di Jepang adalah 35% dan 40% di Amerika Serikat. Operasi ekstraksi katarak bertujuan untuk mengevaluasi segmen posterior dan pada beberapa kasus dapat meningkatkan tajam penglihatan. Tujuan: Melaporkan pendekatan diagnostik dan keberhasilan penatalaksanaan komplikasi katarak bilateral pada sindrom Vogt-Koyanagi-Harada fase konvalesen. Laporan Kasus: Seorang wanita berusia 24 tahun datang ke poliklinik rawat jalan RSUD Dr. Saiful Anwar Malang dengan keluhan penglihatan kabur pada kedua mata yang memburuk 2 bulan sebelum kunjungan, disertai sakit kepala, mual, demam, rambut rontok, dan pemutihan rambut wajah mulai 1 bulan. Pemeriksaan umum menunjukkan alopecia dan poliosis. Tajam penglihatan pada kedua mata adalah 1/300. Pemeriksaan segmen anterior menunjukkan KP coklat polimorfik, flare 4+ dan sel 4+, seklusio pupil, dan katarak pada kedua mata. Pada pemeriksaan USG didapatkan vitritis dan penebalan koroid. Tidak didapatkan kelainan pada pemeriksaan laboratorium. Pasien didiagnosis dengan sindrom Vogt-Koyanagi-Harada fase konvalesen dengan komplikasi katarak dan telah di terapi dengan kombinasi kortikosteroid oral dan azathioprine. Sinekiolisis dan ekstraksi katarak mata kiri dilakukan di bawah perlindungan kortikosteroid oral dan injeksi triamcinolone subtenon posterior. Didapatkan peningkatan tajam penglihatan dan tidak didapatkan rekurensi infeksi pasca operasi. Kesimpulan: Terdapat perbaikan visus mata kiri yang signifikan setelah operasi ekstraksi katarak dengan kontrol inflamasi yang baik sebelum operasi. Tidak ada kekambuhan uveitis selama 6 bulan follow up.
