Backgrounds: Antibiotic resistance can be prevented by controlling the use of antibiotics wisely. Based on WHO recommendations, controlling the use of antibiotics is done by grouping antibiotics with the AWaRe category. Objective: This work aims to present an overview of the use of antibiotics in orthopaedic surgery patients as an effort to monitor the use of antibiotics in a hospital in Bali. Methods: Based on the patient's medical record data, the antibiotics used for orthopaedic surgery were calculated and grouped with the AWaRe classification. Result: Results showed that the most prescribed antibiotic was ceftriaxone (84.74%). There were no antibiotics in the reserve category, but the highest percentage was 94.92% in the watch category, and the access category was 5.08%. Conclusion: There is still non-selective use of antibiotics in this hospital, so in the future, it is necessary to evaluate qualitatively and quantitatively to ensure rational use as well as efforts to prevent the occurrence of antibiotic resistance. Diabetic ulcer is a condition where there are neurological abnormalities and peripheral arterial vascular disease that causes infection, ulceration, and/or damage to the deepest skin tissue on the feet of people with diabetes mellitus (DM). The long duration of wound healing is due to the wrong use of antibiotics for a long time, and bacteria tend to become resistant to antibiotics. Antimicrobial compound fraction of cengkodok combined with gentamicin and ciprofloxacin antibiotics for prevention and reduction of antibiotic resistance in the cause of diabetic ulcers. The drug preparation is in the form of self nanoemulsifiying drug delivery system (SNEDDS), which is the optimum combination of cengkodok leaf fraction with gentamicin and ciprofloxacin antibiotics. The use of SNEDDS in this study is because the SNEDDS made from nanoemulsion formulations has small particle size, large surface absorption, and good dispersion, so that it can be evenly dispersed on the skin and penetrates the skin layer. This study aims to determine the effectiveness of the formulation cengkodok leaf fraction with gentamicin and ciprofloxacin antibiotics against B. cereus and S. aureususing the SNEDDS drug delivery system. The test uses the spotless diffusion method without using paper discs. The results showed that the combination of cengkodok leaf fraction with gentamicin and ciprofloxacin antibiotics had better activity against B. cereus and S. aureus than cengkodok leaf fraction, gentamicin and ciprofloxacin only.

Antimicrobial resistance is now considered a global health problem because it reduces the effectiveness of antimicrobial drugs. According to the World Health Organization (WHO), the highest mortality rate is associated with infections caused by multidrug-resistant microorganisms, with approximately 700,000 deaths worldwide each year. The aim of this study was to determine the potential of C-10 massoia lactone to inhibit the growth of fungi and C. tropicalis biofilm, and molecular docking studies were performed to determine the nature of the inhibition. The study was conducted using the microdilution method for antifungal and antibiofilm testing and designed with a molecular docking approach. Furthermore, an analysis using the scanning electron microscope (SEM) was performed to evaluate the mechanism of effect. The results obtained showed that C-10 massoia lactone can inhibit the growth of fungi by 84.21% w/v. Meanwhile, the growth of C. tropicalis biofilm in the intermediate phase was 80.23% w/v and in the mature phase was 74.23% w/v. SEM results showed that C-10 massoia lactone damaged the EPS matrix of C. tropicalis so that hyphal formation was hindered due to damage to fungal cells, resulting in a decrease in attachment, density, and lysis of C. tropicalis fungal cells. Based on molecular docking tests, C-10 massoia lactone was able to inhibit biofilm formation without affecting microbial growth, while docking C-10 massoia lactone showed a significant binding and has the potential as an antifungal agent. In conclusion, the C-10 massoia lactone compound has the potential as an antibiofilm against C. tropicalis, so it can become a new antibiofilm agent.
